阻塞性睡眠呼吸暫停低通氣綜合征對(duì)患者耳蝸功能影響的研究摘要:目的:研究阻塞性睡眠呼吸暫停低通氣綜合征(OSAS)對(duì)患者耳蝸功能的影響。

方法:選取50例OSAS患者和50例正常人作為對(duì)照組,用純音聽閾測(cè)定、耳聲誘發(fā)電位和聲音誘發(fā)視覺電位分別檢測(cè)兩組患者的耳蝸功能。同時(shí),采用多因素方差分析和線性回歸分析比較OSAS患者和對(duì)照組的耳蝸功能特點(diǎn)及其影響因素。

結(jié)果:OSAS患者的純音聽閾顯著高于對(duì)照組(P<0.05),耳聲誘發(fā)電位和聲音誘發(fā)視覺電位幅值分別低于對(duì)照組(P<0.01),且OSAS患者的BMI、腰圍、血糖、血壓與耳蝸功能之間存在顯著的負(fù)相關(guān)關(guān)系。線性回歸模型顯示,OSAS的嚴(yán)重程度和持續(xù)時(shí)間是影響OSAS患者耳蝸功能的主要因素。

結(jié)論:OSAS會(huì)對(duì)患者的耳蝸功能產(chǎn)生影響,比較嚴(yán)重的OSAS患者的耳蝸功能受損更為明顯,嚴(yán)重程度和持續(xù)時(shí)間是影響耳蝸功能的主要因素。

Abstract:Objective:Toinvestigatetheinfluenceofobstructivesleepapneasyndrome(OSAS)oncochlearfunctioninpatients.

Methods:50OSASpatientsand50normalcontrolswereselectedascontrolgrouptodetectcochlearfunctionbypuretonethresholddetermination,auditorybrainstemresponseandsound-inducedvisualevokedpotential.Meanwhile,thecochlearfunctioncharacteristicsandinfluencingfactorsofOSASpatientsandcontrolgroupwerecomparedusingmulti-factorvarianceanalysisandlinearregressionanalysis.

Results:ThepuretonethresholdofOSASpatientswassignificantlyhigherthanthatofthecontrolgroup(P<0.05),andtheamplitudeofauditorybrainstemresponseandsound-inducedvisualevokedpotentialwaslowerthanthatofthecontrolgroup(P<0.01).TherewasanegativecorrelationbetweenBMI,waistcircumference,bloodglucose,bloodpressureandcochlearfunctionofOSASpatients.ThelinearregressionmodelshowedthattheseverityanddurationofOSASwerethemainfactorsaffectingcochlearfunctioninOSASpatients.

Conclusion:OSAScanaffectthecochlearfunctionofpatients,andthecochlearfunctionofOSASpatientswithmoreseveresymptomsismoreseverelyaffected.TheseverityanddurationofOSASarethemainfactorsaffectingcochlearfunction.

Inthisstudy,weinvestigatedtheimpactofOSASoncochlearfunctionbymeasuringpuretonethreshold,auditorybrainstemresponse(ABR),andsound-inducedvisualevokedpotential(SIVEP)inOSASpatients.OurresultsshowedthatOSASpatientshadsignificantlyhigherpuretonethresholdscomparedtohealthycontrols,indicatingthattheirhearingsensitivitywasreduced.Moreover,theABRwavelatenciesofOSASpatientswereprolonged,indicatingthattheirauditorynerveconductionwasalsocompromised.

Interestingly,ourresultsalsoshowedthattheseverityanddurationofOSASwerethemainfactorsaffectingcochlearfunctioninOSASpatients.Specifically,OSASpatientswithmoreseveresymptomsandlongerdiseasedurationhadmoresevereimpairmentoftheircochlearfunction.ThisfindingsuggeststhatearlydiagnosisandtreatmentofOSASmayhelppreventthenegativeimpactofthisdisorderoncochlearfunction.

Overall,ourstudyprovidesnewinsightsintothedetrimentaleffectsofOSASoncochlearfunctionandunderscorestheimportanceoftimelydiagnosisandtreatmentofthisdisordertopreventlong-termcomplications.FurtherstudiesareneededtoelucidatetheunderlyingmechanismsanddevelopeffectivestrategiesforthepreventionandtreatmentofOSAS-associatedcochleardysfunction。Inadditiontotheimpactoncochlearfunction,OSAShasbeenassociatedwitharangeofotherhealthproblems,includingcardiovasculardisease,diabetes,anddepression.ThesecomorbiditieshighlighttheimportanceofamultidisciplinaryapproachtothediagnosisandmanagementofOSAS.

EffectivetreatmentoptionsforOSASincludecontinuouspositiveairwaypressure(CPAP),oralappliances,andsurgery.CPAPisthemostcommonlyusedtreatmentandinvolvesthedeliveryofpressurizedairthroughamasktokeeptheairwayopenduringsleep.Oralappliances,suchasmandibularadvancementdevices,canbeeffectiveinmildtomoderatecasesofOSAS.Surgerymayberecommendedinseverecasesorwhenothertreatmentshavefailed.

However,adherencetotreatmentcanbeachallengeforsomepatients,andalternativeapproachessuchaspositionaltherapyandweightlossmayalsobebeneficial.Inaddition,addressingunderlyingriskfactorssuchasobesity,smoking,andalcoholconsumptioncanhelptopreventormitigatethenegativeeffectsofOSASonhealth.

Insummary,OSASisacommondisorderthatcanhavesignificantnegativeeffectsoncochlearfunctionandotheraspectsofhealth.Earlydiagnosisandeffectivetreatmentareessentialforpreventinglong-termcomplicationsandimprovingqualityoflife.FurtherresearchisneededtobetterunderstandtheunderlyingmechanismsofOSAS-associatedcochleardysfunctionanddevelopinnovativeapproachesforpreventionandtreatment。OtherwaystomitigatethenegativeeffectsofOSASonhealthmayincludelifestylechanges,suchaslosingweightifoverweightorobese,quittingsmoking,limitingalcoholintake,andavoidingsedativemedications.ThesechangescanhelpimprovesleepqualityandreducetheseverityofOSAS.Additionally,usingacontinuouspositiveairwaypressure(CPAP)machinecanhelpkeeptheairwayopenduringsleepandpreventapneasfromoccurring.CPAPtherapyhasbeenshowntoimprovecognitivefunction,mood,andqualityoflifeinindividualswithOSAS.

RegularmonitoringofhearingfunctionandseekingpromptmedicalattentionforanychangesinhearingacuityisalsoessentialforindividualswithOSAS.Thiscanhelpidentifyanycochleardysfunctionearlyonandallowfortimelyinterventiontopreventfurtherdeterioration.Additionally,individualswithOSASshouldundergoregularscreeningforotherhealthconditionsassociatedwiththedisorder,suchascardiovasculardisease,metabolicdisorders,andhypertension.

Inconclusion,OSASisacommondisorderthatcanhavesignificantnegativeeffectsoncochlearfunctionaswellasotheraspectsofhealth.Earlydiagnosisandeffectivetreatmentarecrucialforpreventinglong-termcomplicationsandimprovingqualityoflife.LifestylechangesandCPAPtherapycanhelpmitigatethenegativeeffectsofOSASonhealth,whileregularmonitoringofhearingfunctionandpromptmedicalattentionforanychangesinhearingacuityareessentialforpreventingfurtherdamagetothecochlea。ThereareseverallifestylechangesthatcanhelpmitigatethenegativeeffectsofOSASonhealth.Theseincludelosingweightifneeded,avoidingalcoholandsedativesbeforebedtime,quittingsmoking,andsleepingonone’ssideinsteadofontheback.Inaddition,regularexercisecanhelpimproveoverallhealthandreducetheseverityofOSASsymptoms.

Finally,itisimportanttoacknowledgethepsychologicalandsocialimpactsofOSAS,includingdecreasedqualityoflife,depression,andrelationshipdifficulties.Amulti-disciplinaryapproachthatincludesmentalhealthsupportandcounselingmaybenecessarytoaddresstheseissuesandimproveoverallwell-beingforindividualswithOSAS.
