1、PAGE9叉車(chē)安全操作程序1.PURPOSE目的Thisprocedureprovidestheminimumrequirementsforthesafeoperationofforklifttrucks.Itenhancesthesafetyofallpersonnelinvolvedintheoperationofforklifttrucks,andalsootherswhomaybeworkingincloseproximitytoforklifttrucks.本程序提供最基本的叉車(chē)安全操作要求。旨在提高叉車(chē)操作人員以及叉車(chē)附近其他工作人員的安全性。2.SCOPE范圍Thisproced

2、ureappliestoallforklifttruckoperationsontheterminalregardlessoftheownershipoftheforklifttrucks.本程序適用于庫(kù)區(qū)范圍內(nèi)所有叉車(chē)操作（無(wú)論何方所有）。3.DEFINITION定義3.1.QualifiedForkliftOperatorisanoperatorwhohasundergonetherequiredtrainingonforkliftoperationsandhasthelicenseissuedbytheinterrelatedauthority.合格叉車(chē)操作人員指經(jīng)相關(guān)叉車(chē)操作培訓(xùn)并擁有

3、相關(guān)管理部門(mén)頒發(fā)的證書(shū)的操作人員。3.2.FlammableLiquidsrefertoproductswithflashpointbelow37.8OC.易燃液體指閃點(diǎn)低于37.8OC的產(chǎn)品。4.RESPONSIBILITIES職責(zé)4.1.Itistheresponsibilityoftheforkliftoperatortoconformtothestipulatedsafetyrequirementsonuseofforklift.叉車(chē)操作人員有責(zé)任遵守與叉車(chē)使用相關(guān)的安全要求。4.2.Itistheresponsibilityofthelinemanagers/supervisorst

4、oensurethatonlytrainedoperatorsareauthorizedtooperateaforklift.直接主管負(fù)責(zé)確保：只有經(jīng)過(guò)培訓(xùn)的有資質(zhì)的人員才能操作叉車(chē)。Theyshallalsoensurethattheforkliftsuppliedbycontractorareperiodicallymaintainedandserviced.Iftheforkliftsaretobeusedforhandlingofflammableliquidindrums,theyshouldbeinstalledwithsparkarrestingdevicessuchasarre

5、stors,flamearrestor,air-startdevice,etc.Inshort,theseforkliftsshallbedesignedforuseinClass1Division2(orZone2)ClassifiedHazardousAreas(refertoSHE-SWP-16).并應(yīng)確保承包商提供的叉車(chē)接受定期維修和保養(yǎng)。若叉車(chē)用于桶裝易燃液體搬運(yùn)，應(yīng)安裝阻火設(shè)備如阻火器、火焰防止器、氣動(dòng)裝置等。簡(jiǎn)而言之，叉車(chē)應(yīng)適用于危險(xiǎn)區(qū)域分類(lèi)1級(jí)2分區(qū)（參見(jiàn)SHE-SWP-16）。4.3.Contractorwhosuppliestheforkliftsisresponsible

6、tomakethenecessaryarrangementsforanyforkliftrepairorperiodicservicingandinspectionbythemanufacturersoastoupkeepthesafeoperabilityoftheforklifttrucks.提供叉車(chē)的承包商負(fù)責(zé)就制造廠(chǎng)家叉車(chē)維修或定期保養(yǎng)、檢查服務(wù)進(jìn)行必要的安排，以維持叉車(chē)的安全可操作性。5.SAFETYREQUIREMENTS安全要求5.1.Allforklifttrucksmustonlybeoperatedbyqualifiedoperators.只有有資質(zhì)的人員才能進(jìn)行叉車(chē)操作。

7、5.2.ForklifttrucksshouldbesuitableforuseinClass1Division2(orZone2)ClassifiedHazardousAreasiftheyareusedforhandlingofflammableliquidsindrums.Themaximumexhaustsurfacetemperatureshouldbeabout200OC(IECT-Code).若用于桶裝易燃液體搬運(yùn)，則叉車(chē)應(yīng)適合在危險(xiǎn)區(qū)分類(lèi)1級(jí)2分區(qū)（參見(jiàn)SHE-SWP-16）中使用。最大表面釋放溫度應(yīng)為200OC（IECT-編碼）。5.3.Alltrafficrulesandw

8、arningsignsontheterminalaretobestrictlyobeyed.嚴(yán)格遵守庫(kù)區(qū)所有交通規(guī)則和警示標(biāo)記。5.4.Ifthereisaneedtooperateaforkliftinhazardousareas,otherthantheroadwaysandDrumYard/Warehouse,theappropriateHotWorkPermitmustbeobtainedfromCCR.若有必要在危險(xiǎn)區(qū)內(nèi)而非道路上或桶堆放場(chǎng)/倉(cāng)庫(kù)內(nèi)進(jìn)行叉車(chē)操作，則必須從中控室取得熱作業(yè)許可證。5.5.Loadlimitsoftheforklifttrucksmustbestencil

9、edonthemastofeachtruckwhereitisclearlyandreadilyvisibletotheoperator.叉車(chē)荷載極限必須印在叉車(chē)升降架上方以便操作人員清楚看到的位置。5.6.Beforeoperatingtheforklifttruckatthebeginningoftheday,theoperatormustinspecttheforklifttrucktoverifythatitisinagoodandsafeworkingconditions.在每天開(kāi)始叉車(chē)操作前，操作人員必須對(duì)叉車(chē)進(jìn)行檢查，以確定其處于良好、安全的工作狀態(tài)。5.7.Theoperato

10、rmustbealertandcautiouswhenoperatingtheforklifttruck.Hemustwatchforothervehicles,equipment,obstacles,roadsurfaceconditionsandpedestrians.操作人員在叉車(chē)操作時(shí)必須保持警覺(jué)和謹(jǐn)慎。操作人員必須注意其它車(chē)輛、設(shè)備、障礙、路面狀況和行人。5.8.Nopassengerisallowedtorideontheforklifttruck.叉車(chē)不許搭載乘客。5.9.Theoperatorshallfaceinthedirectionoftravel.操作人員應(yīng)面對(duì)行駛方向

11、。5.10.Theoperatorshallkeepthemasttiltedbackwithforks150to300mmabovethegroundwhiletravelingloadedorunloaded.行駛中無(wú)論有無(wú)負(fù)重，操作人員應(yīng)將前叉后傾，車(chē)叉距離地面150至300mm。5.11.Theoperatorshallcheckforadequateoverheadandlateralclearancebeforeenteringandduringsubsequentmaneuveringinanybuilding,plantfacilityorcongestedarea.在進(jìn)入任何

12、建筑、裝置設(shè)施或密集區(qū)域之前及之后操作中，操作人員應(yīng)檢查上方及側(cè)面距離是否足夠。5.12.Theoperatorshallsoundthehornatjunctions,cornersandinareaswherevisionisbeingobstructed.在交叉口、轉(zhuǎn)彎處以及視線(xiàn)受阻區(qū)域內(nèi)，操作人員應(yīng)鳴喇叭。5.13.Theoperatorshalldriveslowlyandcautiouslyinthefollowingcircumstances.在以下情況下，操作人員應(yīng)緩慢、謹(jǐn)慎駕駛：whenmakingaturn轉(zhuǎn)彎時(shí)whenreversing倒車(chē)時(shí)drivingoverahum

13、p凹凸不平地面行駛時(shí)drivingupordownaramp上下坡道時(shí)drivingonawet,slipperyorunevensurface濕、滑、不平地面行駛時(shí)drivinginnarrowroads狹窄路面行駛時(shí)drivinginhighnoiseareasorwherevisionisobstructed高噪音區(qū)或視線(xiàn)受阻區(qū)域內(nèi)行駛時(shí)drivingincloseproximitytopotentiallyhazardousarea危險(xiǎn)區(qū)附近行駛時(shí)5.14.Theoperatorshalltravelinthereversemodeiftheloadbeingcarriedobstru

14、ctstheforwardview.若所載重物阻礙前方視線(xiàn)，操作人員應(yīng)倒行。5.15.Theoperatormustalwaysensurethatloadlimitsarenotexceeded.Heshallnotcarryaloadifitisnotproperlysecuredorifitextendsabovethecarriagebackrestunlessnopartoftheloadcanpossiblyslidebacktowardstheoperator.操作人員必須確保不超出荷載極限。若重物未適當(dāng)固定或伸出貨物安全架，操作人員不得承運(yùn)，重物任一部分沒(méi)有向后滑向操作人員的可

15、能時(shí)除外。5.16.Theoperatorshallavoidmakingsuddenstops/startandalso,runningoverlooseobjectsandholeswhichmaybepresentinthepathoftravel.操作人員應(yīng)避免急停急起，并避免在有松軟物體或有坑的路面上行車(chē)。5.17.Forklifttruckshallnotbeusedasaworkingplatformforpeople.Itshallnotbeusedforanypurposeotherthanthatforwhichitwasdesigned.叉車(chē)不得用作人員工作平臺(tái)。除設(shè)計(jì)目

16、的外，不得將叉車(chē)用于其它用途。5.18.Nopersonnelisallowedtostandorwalkundertheelevatedportionoftheforklifttruckwhetherloadedornot.無(wú)論有無(wú)重物，任何人不得在叉車(chē)抬高部分下站立或行走。5.19.Collisions,damagesornearmissesshallbereportedtotheShiftleaderimmediately.若有碰撞、損害或未遂事故，應(yīng)立即報(bào)告當(dāng)班領(lǐng)班。5.20.Noforkliftoperationisallowedonflatbedtrailersorlorries,

17、exceptforstuffingofcontainers.叉車(chē)不得在平板拖車(chē)或卡車(chē)上進(jìn)行操作，集裝箱裝貨時(shí)除外。5.21.Incaseofanincidentinvolvingspillageofflammableliquid,theoperatorshouldattempttoswitch-offtheforkliftengineandnotifyCCR.若易燃液體泄漏引發(fā)事故，操作人員應(yīng)盡量關(guān)閉叉車(chē)發(fā)動(dòng)機(jī)，并通知中控室。5.22.Prolongoperationofengine-drivenforklifttrucksinpoorlyventilatedareasmustbeavoide

18、d.必須避免內(nèi)燃機(jī)叉車(chē)在通風(fēng)不良區(qū)域內(nèi)長(zhǎng)時(shí)間作業(yè)。5.23.Wheneverleftunattended,theoperatorshallparktheforklifttruckinalocationthatdoesnotobstructsafetyorfireprotectionequipmentandroadways.Thefollowingactionsneedtobedone.離開(kāi)叉車(chē)前，司機(jī)應(yīng)將叉車(chē)停在對(duì)安全或消防設(shè)備與通道無(wú)妨礙的位置。需采取以下措施：a.theforksloweredandtiltedforwardsothattheforktipstouchtheground.降

19、下前叉，以便前叉能接觸地面。b.thefunctionalcontrolinneutralmode控制開(kāi)關(guān)在空檔狀態(tài)。c.theparkingbrakeengaged使用停車(chē)制動(dòng)。d.theignitionorkeyswitchturnedoff關(guān)閉點(diǎn)火或鑰匙開(kāi)關(guān)。5.24.Whileoperatingtheforklifttruck,theoperatormustputonsafetyshoes.Hearingprotectiondeviceneedstobeworntoo.進(jìn)行叉車(chē)操作時(shí)，操作人員必須穿上安全鞋，并使用必要的聽(tīng)力保護(hù)用品。5.25.Hosesmaybecarriedbyaforklifttruckiftheyareproperlyenclosedwithinapalletboxwhichissecuredtotheforklifttruck.Carriageofhoseshangingfromtheforksisextremelyunsafeandisnotallow
