19、2 運(yùn)行許可人應(yīng)對(duì)下列事項(xiàng)負(fù)責(zé)4.5.2.1 工作票所列安全措施正確完備，符合現(xiàn)場(chǎng)條件。4.5.2.2 動(dòng)火設(shè)備與運(yùn)行設(shè)備確已隔絕。4.5.2.3 向工作負(fù)責(zé)人交待運(yùn)行所做的安全措施并和工作負(fù)責(zé)人共同到現(xiàn)場(chǎng)檢查安全措施是否已正確地執(zhí)行。4.5.3 工作負(fù)責(zé)人應(yīng)負(fù)責(zé)4.5.3.1 正確安全地組織動(dòng)火工作。4.5.3.2 檢修應(yīng)做的安全措施并使其完善。4.5.3.3 向有關(guān)人員布置動(dòng)火工作,交待防火安全措施和進(jìn)行安全教育。 4.5.3.4 負(fù)責(zé)聯(lián)系化學(xué)分場(chǎng)檢測(cè)人員并提供動(dòng)火分析的取樣點(diǎn)。4.5.3.5 始終監(jiān)督現(xiàn)場(chǎng)動(dòng)火工作。4.5.3.6 辦理動(dòng)火工作票開工和終結(jié)。4.5.3.7 動(dòng)火工作間斷、終

20、結(jié)時(shí)檢查現(xiàn)場(chǎng)無殘留火種。4.5.4 消防監(jiān)護(hù)人應(yīng)負(fù)責(zé)4.5.4.1 動(dòng)火現(xiàn)場(chǎng)配備必要的、足夠的消防設(shè)施。4.5.4.2 檢查現(xiàn)場(chǎng)消防安全措施的完善和正確，現(xiàn)場(chǎng)可燃?xì)怏w和可燃液體的揮發(fā)物含量或粉塵濃度經(jīng)檢測(cè)符合要求，并在動(dòng)火工作票簽字確認(rèn)。4.5.4.3 始終監(jiān)視現(xiàn)場(chǎng)動(dòng)火作業(yè)的動(dòng)態(tài)，發(fā)現(xiàn)火情及時(shí)撲救。4.5.4.4 動(dòng)火工作間斷，終結(jié)時(shí)檢查現(xiàn)場(chǎng)無殘留火種。4.5.5 動(dòng)火執(zhí)行人職責(zé)4.5.5.1 動(dòng)火前必須收到經(jīng)審核批準(zhǔn)且允許動(dòng)火的動(dòng)火工作票。4.5.5.2 按本工種規(guī)定的防火安全要求做好安全措施。4.5.5.3 全面了解動(dòng)火工作任務(wù)和要求，并在規(guī)定的范圍內(nèi)執(zhí)行動(dòng)火。 4.5.5.4 動(dòng)火工作間

21、斷、終結(jié)時(shí)清理并檢查現(xiàn)場(chǎng)無殘留火種。4.5.6 檢測(cè)人員應(yīng)負(fù)責(zé)4.5.6.1 對(duì)其分析手段和結(jié)果負(fù)責(zé)。(燃油氣含量?0.2%，煤粉塵?20g/m3,氫氣含量?3%)4.5.6.2 要親臨現(xiàn)場(chǎng)，按動(dòng)火所在部位負(fù)責(zé)人提供的取樣點(diǎn)逐一取樣分析，在動(dòng)火工作票上如實(shí)填寫取樣時(shí)間，分析數(shù)據(jù)及結(jié)論后簽字。4.5.6.3 分析過的樣品，要保留到該項(xiàng)動(dòng)火作業(yè)結(jié)束。4.6 各級(jí)人員在發(fā)現(xiàn)防火安全措施不完善、不正確或動(dòng)火工作過程中發(fā)現(xiàn)有危險(xiǎn)或違反有關(guān)規(guī)定時(shí)，均有權(quán)立即停止動(dòng)火工作，并報(bào)告上級(jí)防火責(zé)任人。 5 管理內(nèi)容與辦法5.1 動(dòng)火工作票管理標(biāo)準(zhǔn)5.1.1 凡涉及動(dòng)火工作的各級(jí)人員，應(yīng)熟知本管理標(biāo)準(zhǔn)并結(jié)合安全生產(chǎn)

27、處罰，直至追究刑事責(zé)任。5.1.3 動(dòng)火工作系指焊接、氣割和其他一切明火工作(包括使用電爐烘烤、噴燈工作等)。 5.1.4 防火重點(diǎn)部位或場(chǎng)所以及禁止明火區(qū)如需動(dòng)火工作時(shí)，必須執(zhí)行動(dòng)火工作管理標(biāo)準(zhǔn)。5.1.5 根據(jù)火災(zāi)“四大”原則，動(dòng)火工作分為二級(jí)(“四大”指發(fā)生火災(zāi)危險(xiǎn)性大、損失大、傷亡大、影響大)。5.1.5.1 一級(jí)動(dòng)火區(qū):a)油區(qū)及油庫(kù)圍墻內(nèi)。b)油碼頭引橋岸端以內(nèi)。c)輸油、燃油系統(tǒng)及與油系統(tǒng)相連接的汽管道。d)氫氣系統(tǒng)及制氫站圍墻內(nèi)。e)危險(xiǎn)品倉(cāng)庫(kù)及汽油站內(nèi)。f)液化氣站內(nèi)。g)變壓器、蓄電池室、電纜間、電纜溝、電纜隧道、電纜豎井及電纜支架上有可能引燃電纜的動(dòng)火作業(yè)。h)鍋爐制粉系

28、統(tǒng)，指鍋爐的粉倉(cāng)內(nèi)、粉倉(cāng)上方明火作業(yè)有可能造成粉塵爆炸的、絞龍、細(xì)粉分離器等區(qū)域、除此之外的鍋爐制粉系統(tǒng)列為二級(jí)動(dòng)火區(qū)域(確無積粉或設(shè)備解體后確無粉塵飛揚(yáng)者可不開動(dòng)火工作票)。i)汽輪機(jī)油系統(tǒng)。j)易燃易爆物品存放場(chǎng)所;生產(chǎn)現(xiàn)場(chǎng)加油站。k)儲(chǔ)存過可燃?xì)怏w、易燃液體的容器及與此連接的系統(tǒng)和輔助設(shè)備。5.1.5.2 二級(jí)動(dòng)火區(qū):a)與油系統(tǒng)能斷開或加堵板隔離的汽管道。b)油管道支架及其支架上的其他管道。c)氫系統(tǒng)管道及氫冷發(fā)電機(jī)上下周圍30 米范圍內(nèi)。d)有可能造成火花飛濺落至易燃易爆物體(包括電纜)附近的動(dòng)火工作。e)控制室、調(diào)度室、檔案室、計(jì)算機(jī)室、通訊機(jī)房及各種電子間。 5.2 動(dòng)火工作票的

29、填寫5.2.1 動(dòng)火工作票應(yīng)一事、一地辦理，不得兼辦。要用鋼筆或碳素筆填寫，應(yīng)字跡清楚、內(nèi)容正確，不得涂改。5.2.2 “動(dòng)火工作負(fù)責(zé)人”欄的填寫，工作監(jiān)護(hù)人即為動(dòng)火工作負(fù)責(zé)人;一個(gè)工作組動(dòng)火，應(yīng)將該工作組的工作負(fù)責(zé)人姓名填入此欄;幾個(gè)班組進(jìn)行綜合檢修，應(yīng)將總工作負(fù)責(zé)人姓名填入此欄。to find solutions. Grasp the5.2.3 “班組”欄，一個(gè)班組檢修，班組欄填寫工作班組全稱;幾個(gè)班組進(jìn)行綜合檢修，則班組欄填寫檢修單位。5.2.4 “動(dòng)火地點(diǎn)及設(shè)備名稱”欄應(yīng)寫明具體的動(dòng)火工作地點(diǎn)，應(yīng)標(biāo)明設(shè)備名稱。 5.2.5 “動(dòng)火工作內(nèi)容”欄應(yīng)寫明具體的設(shè)備動(dòng)火工作內(nèi)容。5.2.6 “申請(qǐng)動(dòng)火時(shí)間”欄應(yīng)寫明計(jì)劃完成該項(xiàng)動(dòng)火工作所需要的時(shí)間。一級(jí)動(dòng)火工作票的有效時(shí)間不得超過24小時(shí);二級(jí)動(dòng)火工作票的有效時(shí)間不得超過48小時(shí)。 5.2.7 “運(yùn)行應(yīng)采取的安全措施”欄的內(nèi)容措施應(yīng)完整。5.2.7.1 凡動(dòng)火工作需運(yùn)行

35、人員做隔離、沖洗等防火安全措施的，則將措施填寫在此欄內(nèi);否則填寫“無”;嚴(yán)禁此欄為空。5.2.7.2 動(dòng)火設(shè)備與運(yùn)行設(shè)備確已隔斷所采取的安全措施。5.2.7.3 核實(shí)檢修提出的防火和滅火措施是否正確和完善。5.2.7.4 考慮動(dòng)火設(shè)備周邊易燃易爆物品的隔離措施。5.2.8 “檢修應(yīng)采取的安全措施”欄的內(nèi)容措施應(yīng)詳細(xì)完整5.2.8.1 動(dòng)火工作需要的工器具使用、放置措施，如氧氣、乙炔瓶分開放置，相距不小于8米。5.2.8.2 動(dòng)火設(shè)備與易燃易爆物品的隔離措施。5.2.8.3 根據(jù)現(xiàn)場(chǎng)可燃物配備正確的滅火器材的措施。5.2.8.4 現(xiàn)場(chǎng)防火監(jiān)護(hù)措施。5.2.8.5 工作完畢后，清理現(xiàn)場(chǎng)遺留火種的措

36、施。5.2.9 “運(yùn)行應(yīng)采取的安全措施已做完”欄:由工作許可人填寫，應(yīng)填寫“運(yùn)行應(yīng)采取的安全措施”欄內(nèi)提出安全措施在現(xiàn)場(chǎng)的落實(shí)情況，以及現(xiàn)場(chǎng)動(dòng)火設(shè)備與運(yùn)行設(shè)備隔離后的安全措施情況。5.2.10 “動(dòng)火應(yīng)采取的安全措施已做完”欄:由動(dòng)火負(fù)責(zé)人填寫，應(yīng)填寫“動(dòng)火應(yīng)采取的安全措施”欄內(nèi)提出安全措施在現(xiàn)場(chǎng)的落實(shí)情況，以及現(xiàn)場(chǎng)防火和滅火的安全措施情況。5.2.11 “應(yīng)配備的消防設(shè)施和采取的消防措施已符合要求，可燃性、易爆氣體含量或粉塵濃度測(cè)定合格”簽字欄:由化學(xué)測(cè)量人員測(cè)量后在檢測(cè)欄上簽字確認(rèn);動(dòng)火監(jiān)護(hù)人在經(jīng)過上述檢查確認(rèn)后，在消防監(jiān)護(hù)人簽字欄上簽字確認(rèn)。5.2.12 “允許動(dòng)火時(shí)間”欄:由值長(zhǎng)填寫。

37、填寫時(shí)，應(yīng)根據(jù)動(dòng)火現(xiàn)場(chǎng)的實(shí)際情況、動(dòng)火設(shè)備的隔離情況，以及防火和滅火的準(zhǔn)備情況，決定是否準(zhǔn)許動(dòng)火。 5.2.13 “結(jié)束動(dòng)火時(shí)間”欄:由值長(zhǎng)填寫。動(dòng)火工作結(jié)束后，應(yīng)檢查動(dòng)火現(xiàn)場(chǎng)的工作確已結(jié)束，現(xiàn)場(chǎng)確已清理干凈，工作班成員確已撤離現(xiàn)場(chǎng)，無遺留火種，方能辦理動(dòng)火工作票終結(jié)手續(xù)。5.3 動(dòng)火區(qū)域的測(cè)量5.3.1 動(dòng)火申請(qǐng)部門在辦理動(dòng)火工作票前, 應(yīng)根據(jù)動(dòng)火地點(diǎn)、動(dòng)火設(shè)備有無易燃易爆介質(zhì)決定是否需要對(duì)動(dòng)火區(qū)域進(jìn)行測(cè)量。若需要測(cè)量，應(yīng)主動(dòng)與化學(xué)分場(chǎng)檢測(cè)部門聯(lián)系to find solutions. 43、 克/立方米)。b)在油區(qū)內(nèi)、油碼頭引橋端以內(nèi)、油系統(tǒng)的設(shè)備和管道、汽油站內(nèi)、液化氣站內(nèi)、易燃易爆物品存放場(chǎng)所、儲(chǔ)存或儲(chǔ)存過可燃(易燃)氣體(液體)的容器及其連接設(shè)備等處，須在測(cè)定可燃?xì)怏w濃度合格后，方可辦理動(dòng)火工作票手續(xù)，進(jìn)行動(dòng)火作業(yè)，對(duì)可燃?xì)怏w濃度的檢測(cè)由消防人員進(jìn)行(原油氣含量不大于0.2%)。c)在氫氣系統(tǒng)、制氫站內(nèi)、氫冷發(fā)電機(jī)周圍10 米以內(nèi)等處，須在測(cè)定氫氣濃度合格后，方可辦理動(dòng)火工作票手續(xù)，進(jìn)行動(dòng)火作業(yè)，對(duì)氫氣濃度的檢測(cè)由化學(xué)專業(yè)負(fù)責(zé)安排人員進(jìn)行(氫氣含量不大于3%)。5.3.4 一、二級(jí)動(dòng)火工作在次日動(dòng)火前必須重新檢查防火安全措施。測(cè)定可燃?xì)怏w、易燃液體的可燃揮發(fā)物含量或粉塵濃

44、度合格，并經(jīng)安全監(jiān)察部消防專責(zé)認(rèn)可、簽字，方可重新動(dòng)火。4 小時(shí)測(cè)定一次現(xiàn)場(chǎng)可燃?xì)怏w、易燃液體的可5.3.5 一級(jí)動(dòng)火工作的過程中，應(yīng)每隔2 至燃揮發(fā)物含量或粉塵濃度是否合格，當(dāng)發(fā)現(xiàn)不合格或異常升高時(shí)應(yīng)立即停止動(dòng)火，在未查明原因或排除險(xiǎn)情前不得重新動(dòng)火。5.4 動(dòng)火工作必須按照下列原則認(rèn)真執(zhí)行:a)有條件拆下的構(gòu)件，如油管、法蘭等，應(yīng)拆下移至安全場(chǎng)所動(dòng)火。b)可以采用不動(dòng)火的方法代替而同樣能夠達(dá)到效果時(shí)，應(yīng)采用代替的方法處理。c)盡可能地把動(dòng)火的時(shí)間和范圍壓縮到最低限度。5.5 遇到下列情況之一時(shí)，嚴(yán)禁動(dòng)火:a)油船、油車?？康膮^(qū)域。b)壓力容器或管道未泄壓前。c)存放易燃易爆物品的容器未清理

52、火部門(一、二級(jí)動(dòng)火票格式見附件1、附件2)。5.7 動(dòng)火工作票的執(zhí)行5.7.1 動(dòng)火工作票的生成動(dòng)火工作負(fù)責(zé)人根據(jù)動(dòng)火地點(diǎn)及設(shè)備，對(duì)照動(dòng)火級(jí)別的劃分來選擇使用動(dòng)火工作票種類，并負(fù)責(zé)填寫動(dòng)火工作票。填寫內(nèi)容包括工作負(fù)責(zé)人、班組、動(dòng)火地點(diǎn)及設(shè)備名稱、動(dòng)火工作內(nèi)容、申請(qǐng)動(dòng)火時(shí)間、檢修應(yīng)采取的安全措施。5.7.2 動(dòng)火工作票的簽發(fā)動(dòng)火工作負(fù)責(zé)人填票，動(dòng)火工作票簽發(fā)人應(yīng)對(duì)票面內(nèi)容認(rèn)真審核，要考慮動(dòng)火工作的必要性、動(dòng)火工作的安全性、工期的合理性，發(fā)現(xiàn)有誤，返回給工作負(fù)責(zé)人，重新進(jìn)行填寫，確認(rèn)無誤后簽發(fā)動(dòng)火工作票。如果動(dòng)火工作票簽發(fā)人填票時(shí)，動(dòng)火工作票簽發(fā)人還應(yīng)將動(dòng)火工作票全部?jī)?nèi)容向工作負(fù)責(zé)人交待清楚。5
