HemolyticdiseaseofthenewbornItisanisoimmunityhemolysisassociatedwithABOorRhincompatibility.ItresultsfromtransplacentalpassageofmaternalantiboddyactiveagainstRBCantigensoftheinfant,leadingtoanincreasedrateofRBCdestruction.Itisanimportantcauseofanemiaandjaundiceinnewborninfant.

Rhbloodgroupantigens(C,c,D,d,E,e)D>E>C>c>ePathophysiologyofalloimmunehemolysisresultingfromRhincompatibilityAnRh-negativemotherAnRh-positivefetusLeakageoffetalRBCintomaternalcirculationMaternalsensitizationtoDantigenonfetalRBCEtiologyandPathogenesisProductionandtransplacentalpassageofmaternalanti-DantibodiesintofetalcirculationAttachmentofmaternalantibodiestoRh-positivefetalRBCDestructionofantibody-coatedfetalRBCEtiologyandPathogenesisRhhemolyticdiseasewasrareduringthefirstpregnancyinvolvinganRh-positivefetus.Oncesensitizationhasoccurred,re-exposuretoRhDRBCinsubsequentpregnanciesleadstoananamnesticresponse,withanincreaseinthematernalanti-RhDantibodytiter.Thelikelihoodofaninfantbeingaffectedincreasedsignificantlywitheachsubsequentpregnancy.EtiologyandPathogenesisSignificanthemolysisoccurringinthefirstpregnancyindicatespriormaternalexposuretoRh-positiveRBC.FetalbleedingassociatedwithapreviousspontaneousortherapeuticabortionEctopicpregnancyAvarietyofdifferentprenatalproceduresTransfusionofsomeotherbloodproductcontainingRhDRBCinanRh-negativemother
