船/岸安全檢查表SHIP/SHORESAFETYCHECKLIST船名Ship'sName:泊位Berth: 港口Port:抵港日期DateofArrival: 抵港時(shí)間TimeofArrival:填表說(shuō)明/INSTRUCTIONSFORCOMPLETION:安全的作業(yè)需要考慮所有的相關(guān)要求，雙方或單方應(yīng)各自承擔(dān)為遵守這些要求所需的責(zé)任和義務(wù)。無(wú)論哪一方如果未能準(zhǔn)備好接受某一項(xiàng)要求，則應(yīng)在“備注”欄里做好注釋?zhuān)?duì)作業(yè)是否能夠進(jìn)行給予相應(yīng)的評(píng)估。當(dāng)某一具體項(xiàng)被認(rèn)為是不適用于船舶、碼頭或該作業(yè)時(shí)，應(yīng)在“備注”欄里注明。雖然該船/岸檢查表是基于貨物作業(yè)制定的，船舶在碼頭進(jìn)行洗艙作業(yè)時(shí)也建議采取同樣的做法。在“代碼”欄中出現(xiàn)的“A”、“P”和“R”的意思是：A（協(xié)商）—表示該條目應(yīng)在檢查表的“備注”欄里注明所達(dá)成的協(xié)議或議定的程序，或者以其它雙方都認(rèn)可的方式進(jìn)行了溝通。P（許可）—對(duì)于標(biāo)注“P”的條目是否定回答的情況下，未經(jīng)主管機(jī)關(guān)的書(shū)面許可，不可進(jìn)行作業(yè)。R（復(fù)查）—表示該條目在雙方約定和聲明陳述的時(shí)間間隔內(nèi)，需要反復(fù)檢查。聯(lián)合聲明只有當(dāng)各方已經(jīng)進(jìn)行了檢查并且接受相應(yīng)的責(zé)任和義務(wù)后才可簽署。Thesafetyofoperationsrequiresthatallrelevantstatementsareconsideredandtheassociatedresponsibilityandaccountabilityforcomplianceisaccepted,eitherjointlyorsingly.Whereeitherpartyisnotpreparedtoacceptanassignedaccountability,acommentmustbemadeinthe‘Remarks’columnanddueconsiderationshouldbegiventoassessingwhetheroperationscanproceed.Whereaparticularitemisconsideredtobenotapplicabletotheship,theterminalortotheplannedoperation,anotetothiseffectshouldbeenteredinthe‘Remarks’column.WhiletheShip/ShoreCheckListisbaseduponcargohandlingoperations,itisrecommendedthatthesamepracticeisadoptedwhenatankerpresentsitselfataberthfortankcleaning.Thepresenceoftheletters‘A’,‘P’or‘R’inthecolumnentitled‘Code’indicatesthefollowing:A(‘Agreement’).Thisindicatesthatthereferencedconsiderationshouldbeaddressedbyanagreementorprocedurethatshouldbeidentifiedinthe‘Remarks’columnoftheCheckList,orcommunicatedinsomeothermutuallyacceptableform.P(‘Permission’).Inthecaseofanegativeanswertothestatementscoded‘P’,nooperationsaretobeconductedwithoutthewrittenpermissionfromtheappropriateauthority.R(‘Re-check’).Thisindicatesitemstobere-checkedatappropriateintervals,asagreedbetweenbothpartiesandstatedinthedeclaration(Suggestthere-checkintervalsdonotexceed2hours).Thejointdeclarationshouldnotbesigneduntilallpartieshavecheckedandacceptedtheirassignedresponsibilitiesandaccountabilities.
