7、014年12月25日。委托重慶市弘禹水利咨詢有限公司主持對。埋石 混凝土重力壩工程左岸非溢流壩段分部工程進行驗收，分部丄程驗收匸作組通過察看現 場、查閱資料，（聽取施工、設計、監(jiān)理等單位的工作報告）討論并通過該 分部工程驗 收鑒定書。一、分部工程開工完工日期：本分部工程于2012年5月23日開工，至2013年8月26日完工。二、分部工程建設內容本分部工程建設內容為:防滲面板栓、壩體埋石栓、墊層栓及換填栓澆筑、（壩內 預埋鋼筋栓管集水道、PVC排水管核實有無此部工程量）瀝青杉木板制安等。三、施工過程及完成的主要工程量：1、施工過程：本分部工程于2012年5月23日開始（澆筑）18號壩段、2012

8、年12月7日開始 9號壩段換填栓（9號壩段無換填栓）、墊層栓、防滲栓及壩體埋石磴施工，于2013年 8月26日全部完成。施工前及過程中對建筑物輪廓線及線型變化點進行了施丄放樣， 并將相關成果報監(jiān)理工程師復核。外露面模板使用定型鋼模加拉筋固定，沉降縫模板使 用木模拼裝而成，同時按照設計要求設置瀝青杉木板沉降縫及上游防滲面板的銅片和橡 膠止水。在模板安裝及基礎面或施丄縫處理等工序驗收合格并經監(jiān)理工程師簽署栓開倉 許可證后進行栓澆筑，栓及埋石采用塔機吊運入倉。栓 澆筑嚴格執(zhí)行“三檢制”及監(jiān)理 “旁站制”，做到每個施工工療;經監(jiān)理工程師驗收 合格后方進行下一工序的施工。在施匸過程中按照設訃及規(guī)范要求對

9、基礎面或施工縫、模板安裝、預埋件制作 及安 裝、栓澆筑、埋石及栓外觀質量等工序進行檢查、控制，同時加強了（碗試配）原材料 及中間產品的檢測取樣檢測工作。通過對施工工序及原材料、中間產品的有 效控制，本 分部工程質量滿足設計及規(guī)范要求。2、完成的主要工程量：壩基C15換填栓約3800m?, C15W4墊層栓約5100m?, C20W6防滲栓約7500m?, C15 埋石碗約31100m?,預制栓管、PVC管。四、質量事故及缺陷處理：無。五、擬驗工程質量評定：1、單元工程劃分根據重慶市水利工程質量監(jiān)督中心站以渝水質監(jiān)。號文件對。工程單 位、分部工程批復，施工單位對單元工程進行細化，監(jiān)理審核，業(yè)主認

15、止水等原材料檢測28組，C15抗壓強度檢測34組，C20抗壓強度檢測32組，各項檢測指標均滿足設計 及規(guī)范要求。3、施工單位自評結果本分部工程共351個單元工程，根據水利水電工程施工質量檢驗與評定規(guī)程 SL176-2007,施工單位自評單元工程質量全部合格，合格率100%,其中優(yōu)良單元工程114 個，單元工程優(yōu)良率為32. 5%;自評質量等級：合格。4、監(jiān)理單位復核意見經復核351個單元工程質量全部合格，合格率100%,其中優(yōu)良單元工程114個，單 元工程優(yōu)良率32. 5%;原材料及中間產品質量合格，資料基本齊全。復核質量等級:合 格。5、分部工程質量等級評定意見本分部工程共351個單元工程，

16、原材料及中間產品質量合格，施工中無質量事故、 質量缺陷和安全事故;單元工程經施工單位自評質量等級合格，監(jiān)理單位復核質量等級 合格，項LI法人認定質量等級合格。分部工程質量等級評定意見:合格。六、驗收遺留問題及處理意見：無。七、驗收結論：依據水利水電建設工程驗收規(guī)程(SL223-2008)要求，驗收組通過現場查看、核 查資料后一致認為，該分部工程已按設計及合同約定全部完成施工，施工工 藝符合栓規(guī) 范。施工后的質量符合設計及規(guī)范要求，資料基本齊全，施工中未發(fā)生質量和安全事 故。本分部工程共劃分為351個單元工程，質量全部合格，合格率100%,其中優(yōu)良單元 工程為114個，單元工程優(yōu)良率32. 5%
