1、1施工作業(yè)HSE管理HSE Management in Construction Works1.1 施工作業(yè)前的HSE管理HSE management before construction works單項(xiàng)工程或單位工程開工前的HSE檢查確認(rèn)包括以下內(nèi)容：Before starting a sub-project or a unit works, HSE inspection shall be performed to confirm the following:（1）安全的教育培訓(xùn)記錄；Education and training record on safety;（2）安全協(xié)議書；Agre

2、ement on safety;（3）施工方案中的風(fēng)險(xiǎn)分析和防范措施；Risk analysis and prevention measures in construction plan;（4）安全防護(hù)措施的準(zhǔn)備和落實(shí)情況；Preparation and implementation of safety protection measures;（5）即將投入使用的機(jī)具、設(shè)備的性能和完好性檢驗(yàn)、試驗(yàn)記錄（出廠合格證、維護(hù)保養(yǎng)記錄、試驗(yàn)記錄、檢定證明或年檢證明）；Inspection and testing records on performance and soundness of machi

3、nery and equipment to be put into operation (ex-works certificate, maintenance record, testing record, calibration certificate or annual inspection certificate);（6）個(gè)人防護(hù)用品的配備及其符合性；Provision of individual protection articles and their conformity;（7）現(xiàn)場救護(hù)所必備的設(shè)施、器材和人員的落實(shí)和到位情況（救護(hù)設(shè)施的合格證、救護(hù)人員資質(zhì)證書）；Provision

5、operators and scaffolding workers;（9）作業(yè)許可證的辦理情況，如動(dòng)火作業(yè)票、動(dòng)土作業(yè)票等；Availability of work permits, such as permit for works with fire and permit for earthworks;（10）作業(yè)環(huán)境；Working environment;（11）作業(yè)進(jìn)行過程中安全員落實(shí)情況。Assignment of safety persons during works.施工作業(yè)前，施工員對(duì)參加施工作業(yè)人員進(jìn)行施工作業(yè)技術(shù)及要求交底，根據(jù)作業(yè)前對(duì)總的施工步驟中存在的危險(xiǎn)所采取的控制措

6、施，安全員應(yīng)負(fù)責(zé)進(jìn)行施工作業(yè)HSE管理交底，辦理相關(guān)的工作許可證；Before starting the works, construction controllers shall give detailed instructions of the work technical requirements to construction workers, and according to the control measures to be adopted with respect to dangers existing in the general work steps before the

7、works, safety persons shall be responsible for instructions on HSE management for the works and obtaining relevant work permits;作業(yè)前HSE檢查分為裝置區(qū)外和裝置區(qū)內(nèi)檢查，在開始作業(yè)前，由項(xiàng)目部組織，各級(jí)專業(yè)負(fù)責(zé)人參加，如CTCI有要求也可以參加，按照相關(guān)檢查內(nèi)容進(jìn)行檢查。The HSE inspections before work are divided into inspections within the plant area and inspections

8、out of the plant area. They will be organized by the Project Management before starting the works, and will be participated by responsible persons for various specialties, and also by CTCI when requiring, and inspections will be performed for relevant items.施工作業(yè)前應(yīng)對(duì)作業(yè)環(huán)境進(jìn)行檢查，每日應(yīng)進(jìn)行針對(duì)性的班前講話，做到“三交一清”（即交待

9、作業(yè)內(nèi)容、交待作業(yè)環(huán)境、交待作業(yè)中存在的危險(xiǎn)因素和風(fēng)險(xiǎn)，清楚職工的身體和思想動(dòng)態(tài)），并做好記錄。Before the construction works, the work environment shall be inspected. Every day, speech on relevant issues should be made before work, to make clear the work to be performed, the work environment, dangerous factors and risks in the works and the phys

10、ical and ideological conditions of employees (“making four aspects clear”), and all these will be properly recorded.對(duì)于在檢查時(shí)發(fā)現(xiàn)的HSE隱患，項(xiàng)目部將給施工責(zé)任方下發(fā)隱患整改通知單，要求施工方盡快整改，施工方整改完并經(jīng)自檢合格后，填寫隱患整改情況反饋單報(bào)項(xiàng)目部，由項(xiàng)目部再組織并按上述檢查程序進(jìn)行檢查和確認(rèn)。For any HSE hidden peril found during inspections, the Project Management shall issue

11、the “Rectification Notice” to the relevant work unit, requesting prompt rectification. After completion of rectification and qualification by self-inspection, the work unit shall fill in the “Feedback sheet on rectification” and submit it to the Project Management, which will then organize inspectio

12、ns and confirmation according to the above-mentioned inspection procedures.1.2 施工作業(yè)中的HSE管理HSE management during construction works工程施工必須按照施工技術(shù)方案和相應(yīng)施工工藝技術(shù)要求組織施工作業(yè)；Construction works must be organized according to the construction technical scheme and corresponding construction process requirements;施工

13、中施工人員應(yīng)嚴(yán)格按相關(guān)規(guī)定、制度和本工種的安全操作規(guī)程進(jìn)行作業(yè)；During works, construction personnel must strictly follow relevant rules and regulations and safety operation procedures for the jobs;施工中安全員應(yīng)嚴(yán)格執(zhí)行現(xiàn)場監(jiān)督程序，嚴(yán)格執(zhí)行隱患治理程序；During works, safety persons shall strictly implement site supervision procedures and procedures to contr

14、ol and prevent hidden perils;施工中設(shè)備設(shè)施和使用的勞動(dòng)保護(hù)用品應(yīng)嚴(yán)格按相關(guān)規(guī)定配備和管理；During works, the corresponding configuration and management rules shall be strictly followed for equipment and facilities and the use of labor protection articles;施工中施工人員應(yīng)確保作業(yè)周圍的人員、設(shè)施及物品處于安全狀態(tài)，環(huán)境不被污染；During works, construction personnel sh

17、tion articles used on the worksite shall be counted;施工作業(yè)后應(yīng)清點(diǎn)所有施工作業(yè)人員；After the work, all working personnel shall be counted;施工作業(yè)后應(yīng)檢查本作業(yè)結(jié)果是否對(duì)下道工序存在隱患，有則改之；After the work, inspection shall be made on whether there is any hidden peril from the current work to the subsequent step of work, and any such p

19、hall be followed:1.4 施工作業(yè)HSE管理規(guī)章制度HSE management systems and regulations for construction works1.4.1 HSE會(huì)議制度HSE meeting system1.4.1.1 項(xiàng)目部會(huì)議制度Project Management meeting system會(huì)議的分類與時(shí)間：項(xiàng)目部的HSE會(huì)議分四類,即Classification and timing of meetings: there are four types of HSE meetings for the Project Management:

20、 （1）HSE每周例會(huì)：與每周的最后一個(gè)工程協(xié)調(diào)會(huì)一并召開；HSE weekly routine meetings: held together with the last project coordination meeting of every week;（2）HSE專題會(huì)：根據(jù)施工現(xiàn)場HSE的實(shí)施情況和存在問題，確定會(huì)議時(shí)間，但每月至少一次；HSE special topic meetings: the meeting time will be determined according to the HSE implementation condition and existing pr

21、oblems on work site, but at least once a month;（3）全體人員每周一次TBM：每周一上午開始工作前召開；TBM for all personnel once a week: held before work on every Monday morning;（4）全體人員每天一次安全講話：每天上午開始工作前召開。Safety speech for all personnel every day: before work every morning.在項(xiàng)目部組織的每周工程協(xié)調(diào)會(huì)上，有關(guān)HSE的內(nèi)容作為會(huì)議的一項(xiàng)議程，主要內(nèi)容有：由項(xiàng)目部的HSE經(jīng)理通報(bào)

22、上周的HSE實(shí)施情況、檢查結(jié)果、存在問題；布置、安排下階段施工現(xiàn)場HSE的工作計(jì)劃和實(shí)施的要求；提出持續(xù)改進(jìn)的要求；傳達(dá)上級(jí)部門、CTCI、的有關(guān)HSE管理的文件、指示、通報(bào)等；At the weekly project coordination meetings organized by the Project Management, the relevant HSE issues will be an agenda of the meeting, and they mainly include: the HSE manager of Project Management reporting

23、 the HSE implementation, inspection results and existing problems during the previous week; allocating and arranging HSE work plan and implementation requirements on worksite for the subsequent stage; proposing requirements on continual improvement; conveying relevant documents, instructions and cir

24、culars on HSE management from the superior authorities and CTCI;在每周工程協(xié)調(diào)會(huì)上，對(duì)施工現(xiàn)場的HSE實(shí)施情況和存在的問題，要進(jìn)行全面總結(jié)，對(duì)下一步的工作要進(jìn)行詳細(xì)布置。At the weekly project coordination meetings, overall summary will be made for the HSE implementation and existing problems on the worksite, and detailed arrangement will be made for s

25、ubsequent works.項(xiàng)目部全部或以二級(jí)施工單位為單位每周一上午 7:308:00之間進(jìn)行TBM，項(xiàng)目部領(lǐng)導(dǎo)按要求參加定點(diǎn)聯(lián)系的二級(jí)施工單位的安全活動(dòng)，由部門和施工隊(duì)的專職或兼職安全員分別組織召開并做好記錄；也可以是全體人員集中在一個(gè)地點(diǎn)，由HSE經(jīng)理組織召開。其主要內(nèi)容是：TBM will be held at 7:308:00 every Monday morning for all personnel in the Project Management or by units of level II work units, and Project Management

26、 leaders will participate in the safety activities of the designated contact level II work units as required. This meeting will be organized by full-time or part-time safety persons of the department or work team, and records will be made. This meeting can also be organized by the HSE manager for al

27、l personnel at one location. The main contents are:（1）上周HSE實(shí)施情況簡單介紹；Brief presentation of HSE implementation during the previous week;（2）本周施工時(shí)的有關(guān)HSE要求和注意事項(xiàng)；Relevant HSE requirements and precautions for work during the present week;（3）工作環(huán)境狀況、安全防護(hù)措施及施工注意事項(xiàng)；Conditions of working environment, safety pro

28、tection measures and precautions in works;（4）所使用的設(shè)備狀況及操作注意事項(xiàng)；Conditions of equipment used and operation precautions;（5）個(gè)人防護(hù)要求和應(yīng)急準(zhǔn)備。Requirements on individual protection and preparation for emergency.項(xiàng)目部HSE專題會(huì)的組織和內(nèi)容：這種會(huì)議主要是由項(xiàng)目部安全部組織召開，正常情況下，是在每月的最后一周召開，其會(huì)議主要內(nèi)容是：Organization and content of special top

29、ic HSE meetings of Project Management: such meetings will mainly be organized by the Safety Department of Project Management, and will be arranged in the last week of each month under normal conditions. The main contents of the meeting are:（1）對(duì)二級(jí)施工單位一個(gè)月的HSE實(shí)施情況進(jìn)行評(píng)比；Assessing the HSE implementation o

30、f level II work units during the month;（2）指出存在的不足之處并提出持續(xù)改進(jìn)的要求；Identifying deficiencies and proposing requirements on continual improvement;（3）布置下一階段的工作。Arranging the work for the subsequent stage.但如果因HSE的實(shí)施過程中出現(xiàn)了問題且有可能繼續(xù)發(fā)生的趨勢，在這種情況下，項(xiàng)目部也將及時(shí)組織召開專題會(huì)，及時(shí)解決存在的問題或?qū)⒁霈F(xiàn)的問題。其主要內(nèi)容是：However, if there is any pr

31、oblem in the implementation of HSE and there is the trend of continuous occurrence, the Project Management may organize a special topic meeting to solve the existing or foreseen problem in a timely manner. The main contents are:（1）專題事項(xiàng)的詳細(xì)情況及存在問題的介紹；Presentation of details of the matter covered by th

32、e special topic and existing problems;（2）針對(duì)專題事項(xiàng)或存在問題的，制定糾正措施；Working out corrective actions for the matter or existing problems;（3）為了預(yù)防問題的再發(fā)生，制定必要的預(yù)防措施。Working out necessary preventive actions to prevent recurrence of the problem.1.4.1.2 基層班組會(huì)議制度Meeting system for grass-root teams班組除參加項(xiàng)目部周一安全活動(dòng)外，班組長

33、每天早上按“三交一清”的要求組織召開班前會(huì)，對(duì)每天的工作做出分工和安排，對(duì)重點(diǎn)部位、環(huán)節(jié)、工序及崗位進(jìn)行教育作好交待，并做好班前講話記錄；In addition to the safety activity arranged by the Project Management every Monday, the team leaders shall organize a meeting before work every morning according to the requirements of “making four aspects clear”, to arrange the wo

35、g safety person, mainly for summarizing the safety conditions of the previous day, make instructions on possible dangers to be involved in the work of the day to the charged units according to the risk evaluation report, and pointing out emphasis of protection in the work.1.4.2 HSE檢查、考核、獎(jiǎng)懲管理制度HSE in

36、spection, examination, award and punishment regulations1.4.2.1 HSE檢查管理制度HSE inspection and management regulations項(xiàng)目部檢查以定期檢查、專項(xiàng)檢查、日常檢查和節(jié)假日檢查四種形式相結(jié)合，每月根據(jù)施工重點(diǎn)與HSE動(dòng)向開展專項(xiàng)檢查；The Project Management will conduct inspections in the combination of four types: periodical inspection, special inspection, daily i

37、nspection and inspection during holidays, and conduct special topic inspections according to emphasis of works and HSE movements;日常檢查由現(xiàn)場安全員完成，負(fù)責(zé)建立健全巡檢日記，對(duì)隱患（問題）提出整改措施，對(duì)違章提出處罰建議；Daily inspection shall be performed by site safety persons, who will establish and keep patrol inspection logs, propose cor

38、rection and rectification measures on hidden perils (problems) and propose on punishment to violation of rules;定期、專項(xiàng)及節(jié)假日檢查由HSE經(jīng)理組織，安全員參加，定期每月一次，專項(xiàng)可根據(jù)工程特點(diǎn)安排有針對(duì)性的檢查；Periodical and special inspections and inspections during holidays shall be organized by HSE manager and participated by safety persons.

39、Periodical inspections shall be performed once a month, and special inspections can be arranged for specific issued according to characteristics of the works;檢查計(jì)劃在每月安全部的工作計(jì)劃中體現(xiàn)，每次檢查最后分類匯總，形成檢查記錄，并下發(fā)隱患整改單、隱患整改備忘錄或安全工作聯(lián)系箋到相關(guān)單位；The inspection program shall be indicated in the monthly working plan of Sa

40、fety Department. For each inspection, summary will be made by categories to form an inspection record, and Rectification Notice and Rectification Memorandum will be issued, or safety work liaison sheets be sent to relevant units;現(xiàn)場安全員對(duì)受檢單位的隱患整改情況進(jìn)行檢查監(jiān)督，受檢單位整改完后向安全部提出復(fù)查申請(qǐng)，由安全部負(fù)責(zé)驗(yàn)收；Site safety persons

41、 shall check and supervise the correction and rectification of hidden perils in the inspected units, and after completion of correction and rectification, the inspected units shall submit a request for re-inspection to the Safety Department, who will be responsible for acceptance.1.4.2.2 HSE考核管理制度HS

42、E examination and management regulations根據(jù)項(xiàng)目部對(duì)二級(jí)施工單位和部門每月考核規(guī)定，組成安全考核小組對(duì)各單位考核。A safety examination group will be organized to examine various units according to the regulation of Project Management on monthly examination of level II work units and departments.將各項(xiàng)HSE檢查的結(jié)果與考核相結(jié)合，即定期考核與日常檢查考核結(jié)合，定期考核每月進(jìn)行

43、兩次，分別為中旬與月底考核。The results of various HSE inspections will be combined with examination, i.e. periodical examination will be combined with daily inspections and examinations. Periodical examination will be performed twice a month, respectively in the middle and at the end of the month.對(duì)每月考核結(jié)果進(jìn)行匯總，評(píng)比分

44、并與獎(jiǎng)金掛鉤，安全風(fēng)險(xiǎn)抵押金的返回比例及年終評(píng)優(yōu)考核應(yīng)參照月考核結(jié)果。The examination results of each month will be summarized, scored and be linked with bonus. The proportion of return of safety and risk mortgage money and year end excellence examination will also make reference to the monthly examination results.1.4.2.3 HSE獎(jiǎng)懲管理制度HS

45、E award and punishment management system項(xiàng)目部內(nèi)部獎(jiǎng)懲執(zhí)行公司安全管理規(guī)定；Within the Project Management, the “Regulation on safety management” of the company will be implemented.所有違章者被CTCI發(fā)現(xiàn)而通報(bào)的，項(xiàng)目部將按規(guī)定對(duì)該單位進(jìn)行雙倍的處罰， CTCI發(fā)現(xiàn)而處罰，所有處罰額全部由該違章單位承擔(dān)，此外項(xiàng)目部對(duì)該單位進(jìn)行雙倍的追加處罰。For any violation of rule found and reported by CTCI, th

46、e Project Management will double the punishment of the unit concerned according to rules. When violation is found and punished by CTCI, all penalty month will be borne by the violating unit, and in addition, the Project Management will give additional double punishment on such a unit.對(duì)違章嚴(yán)重的二級(jí)施工單位，安全

47、部將要求該單位及個(gè)人進(jìn)行停工處理，直到整改合格為止，情節(jié)嚴(yán)重者，安全部將勒令其退出項(xiàng)目部，但以上措施不妨礙且也不替代任何二級(jí)施工單位或個(gè)人因違犯施工管理程序或違章作業(yè)而導(dǎo)致的財(cái)產(chǎn)損失及人員傷害等重大HSE事故所受到的法律責(zé)任或處罰。For any level II work unit having seriously violated rules, the Safety Department shall require that unit and individuals to stop work for disposition, until qualification in correctio

48、n and rectification. Those with serious cases shall be ordered by the Safety Department to leave the Project Management. However, these actions will not hinder nor substitute any legal liability or penalty against any level II work unit or individual on property damage and personal injury or other m

49、ajor HSE accidents resulted from violation of construction management procedure or from working in violation of rules.1.4.3 HSE事故報(bào)告和信息交流制度HSE accident report and information exchange system發(fā)生事故應(yīng)按CTCI要求向有關(guān)單位報(bào)告事故；Any accident shall be reported to relevant organizations as required by CTCI;信息主要來自公司、CTC

50、I、和兄弟單位及現(xiàn)場信息綜合；Information shall mainly be sources from the company, CTCI and peer organizations and site information summary;1.4.4 環(huán)境保護(hù)Environment protection1.4.4.1 環(huán)境影響評(píng)價(jià)Evaluation on environmental impact項(xiàng)目開工前項(xiàng)目部應(yīng)組織相關(guān)人員對(duì)施工現(xiàn)場和生活區(qū)進(jìn)行可行性評(píng)估，做出評(píng)估報(bào)告；Before starting work for the project, Project Managemen

51、t shall organize relevant personnel to conduct a feasibility evaluation of the construction site and living quarter, and prepare an evaluation report;在施工過程項(xiàng)目部相關(guān)職能部門中對(duì)施工作業(yè)環(huán)境進(jìn)行相關(guān)內(nèi)容的檢測，以便對(duì)下一步改善作業(yè)環(huán)境提供基本的數(shù)據(jù)；During the work, the relevant functional departments of Project Management shall make testing on r

52、elevant items on the work environment, so as to provide basic data for further improving the work environment;在發(fā)生環(huán)境污染事故時(shí)，應(yīng)按應(yīng)急程序進(jìn)行對(duì)環(huán)境的評(píng)價(jià)。In case of any accident with pollution to the environment, environmental evaluation shall be conducted according to emergency procedures.1.4.4.2 環(huán)境保護(hù)措施Environment p

53、rotection measures職工要注意個(gè)人衛(wèi)生和飲食衛(wèi)生，預(yù)防疾病發(fā)生；Employees shall take care of individual sanitation and food sanitation, to prevent disease;施工現(xiàn)場道路在指定專人清掃的同時(shí)，還應(yīng)定期灑水，防止塵土飛揚(yáng)；Roads on work site will be sprayed with water periodically to prevent dust from rising, in addition to cleaning by designated personnel;水泥

54、、白灰等易飛揚(yáng)的細(xì)顆粒材料，應(yīng)在庫內(nèi)存放，室外露天存放時(shí)必須下墊上蓋防止揚(yáng)塵；Materials with fine particles likely to rise, such as cement and white ash, shall be stored indoor, or covered when stored outdoor, to prevent dust rising;下雨時(shí)，在工地大門處鋪墊草袋，在雨后泥濘的情況下及時(shí)清理車輛輪胎上上的泥土；During rain, straw bags shall be laid at the site gate, and mud on v

55、ehicle wheels will be cleaned promptly when it is muddy after rain;禁止在施工現(xiàn)場焚燒雜草和各種垃圾；It is forbidden to burn weeds and rubbish at worksite;現(xiàn)場噴砂除銹施工時(shí)應(yīng)采取相應(yīng)措施減少粉砂和鐵銹的飛揚(yáng)；During rust removal by blasting in field, relevant provisions shall be made to reduce flying dust and rust;施工現(xiàn)場對(duì)CTCI、的綠化要愛護(hù)，花草、樹木確因施工需要

56、遷移的，要與CTCI、協(xié)商并采取措施后方可施工；Care shall be taken for the plantation of CTCI on site, and when it is really necessary to relocate flowers and trees due to construction, consultation shall be made with CTCI and measures be taken before proceeding;為防止水質(zhì)污染，施工中禁止將施工用的酸洗液、鈍化液未經(jīng)中和處理就直接排放。To prevent pollution to

57、 water, it is forbidden to directly discharge pickling liquid and passivation liquid without neutralization during works.施工中禁止將有毒有害廢棄物作土方回填；It is forbidden to use toxic or harmful wastes as earth for backfilling;理化用的化學(xué)藥品應(yīng)集中庫內(nèi)存放，要加蓋保管，防止灑落污染水質(zhì)；Chemicals for physical and chemical tests shall be stored

58、 in warehouses in a concentrated manner, and be stored with covers to prevent spreading to pollute water;施工用現(xiàn)場砼攪拌站的廢水須經(jīng)專門的沉淀后再排放；現(xiàn)場汽車修理，動(dòng)設(shè)備拆解用的廢油應(yīng)集中收集，統(tǒng)一處理，嚴(yán)禁亂排放；職工食堂污水排放時(shí)應(yīng)設(shè)置隔油池定期淘油和清除雜物，防止直接排放污染水質(zhì)；Wastewater from concrete batching plant on worksite must be allowed to settle before discharge; waste oil for vehicle repair and dismantling of equipment on site will be collected and treated on a unified basis, an
