Interms ofprojectexamination andapproval,ndot herde partments, "one-stoprece ption, one-stopservice",the simplifie臨床醫(yī)學(xué)專業(yè)（全科醫(yī)學(xué)方向）學(xué)科發(fā)展規(guī)劃一、專業(yè)開設(shè)背景和經(jīng)過1）背景我國衛(wèi)生人才資源分布不均衡、結(jié)構(gòu)失調(diào)且現(xiàn)行培養(yǎng)模式與需求不協(xié)調(diào)，過度集中在大城市和大醫(yī)院，農(nóng)村社區(qū)衛(wèi)生人才短缺，總體素質(zhì)不高，特別是全科醫(yī)學(xué)人才嚴重匱乏，由此造成城鄉(xiāng)之間和地區(qū)之間醫(yī)療衛(wèi)生發(fā)展不平衡，農(nóng)村社區(qū)衛(wèi)生發(fā)

8、展嚴重滯后，人民群眾看病難、看病貴，居民健康狀況差距擴大。為此， 中共中央、國務(wù)院關(guān)于衛(wèi)生改革與發(fā)展的決定做出了“加快發(fā)展全科醫(yī)學(xué)，培養(yǎng)全科醫(yī)生”的重要決策。實現(xiàn)這一戰(zhàn)略任務(wù)，加快發(fā)展全科醫(yī)學(xué)教育， 建設(shè)一支以全科醫(yī)師為骨干的高素質(zhì)的社區(qū)衛(wèi)生服務(wù)隊伍，保證社區(qū)衛(wèi)生服務(wù)深入、健康、持續(xù)發(fā)展，是高等醫(yī)學(xué)院校肩負的重任。根據(jù)這一特殊國情的迫切需要，我院審時度勢，在衛(wèi)生部陳竺部長的親自指示、 關(guān)懷和支持下，緊密結(jié)合我國當(dāng)前現(xiàn)有國情和我院實際辦學(xué)能力，順應(yīng)天時、地利、 人和， 創(chuàng)造性的提出了在臨床醫(yī)學(xué)專科起點的基礎(chǔ)上培養(yǎng)定位于廣大城鎮(zhèn)社區(qū)和農(nóng)村社區(qū)的全科醫(yī)學(xué)教育，為廣大農(nóng)村和城鎮(zhèn)社區(qū)培養(yǎng) “下得去、 留

9、得住、用得上”的合格的實用型全科醫(yī)生。2）經(jīng)過（ 1）成立全科醫(yī)學(xué)課題小組我院領(lǐng)導(dǎo)班子對該項工作高度重視，并為此專門成立全科醫(yī)學(xué)課題小組，由一名副院長主抓該項工作，課題組成員是由我院的高學(xué)歷、高職稱、 高水平的業(yè)務(wù)骨干組成。（2）出外學(xué)習(xí)考察調(diào)研課題組分期、分批到浙江大學(xué)醫(yī)學(xué)院、首都醫(yī)科大學(xué)、徐州醫(yī)學(xué)院等有豐富的全科醫(yī)學(xué)教育經(jīng)驗的醫(yī)學(xué)院校學(xué)習(xí)、考察， 初步確立了適合我國國情和我院實際的專科起點的全科醫(yī)學(xué)教育的培養(yǎng)目標(biāo)和培養(yǎng)模式，對課程設(shè)置、教學(xué)大綱的制訂、師資培訓(xùn)、考核體系等方面也作了全面細致的調(diào)研。此外，課題組還明確分工， 一部分成員深入到國內(nèi)著名的多所三級?？漆t(yī)院（如北京友誼醫(yī)院、復(fù)興醫(yī)院

10、等） 及其各級社區(qū)衛(wèi)生服務(wù)中心及站點，調(diào)研全科醫(yī)學(xué)生在?？漆t(yī)院和社區(qū)中心的臨床實習(xí)及社區(qū)實踐情況；另一部分成員則深入到當(dāng)?shù)匦l(wèi)生行政部門和各社區(qū)服務(wù)中心（站點）廣泛調(diào)查，取得第一手資料，以本地狀況作為縮影，初步掌握當(dāng)前我國城鎮(zhèn)社區(qū)和廣大農(nóng)村的衛(wèi)生人才資源現(xiàn)狀、老百姓的健康狀況及其對衛(wèi)生健康需求狀況等（詳見調(diào)查報告）。根據(jù)以上調(diào)研，課題組及時寫出在我院開設(shè)定位于城鎮(zhèn)社區(qū)及廣大農(nóng)村的?？破瘘c臨床醫(yī)學(xué)專業(yè)（全科醫(yī)學(xué)方向）的可行性報告。課題組的各項調(diào)研工作得到了衛(wèi)生部、省衛(wèi)生廳領(lǐng)導(dǎo)和國內(nèi)知名醫(yī)學(xué)院校的相關(guān)專家以及我市衛(wèi)生行政部門的高度評價和大力支持。（3）制訂教學(xué)計劃、教學(xué)大綱和專家論證在最短的時間內(nèi)，

Promoting threh newdevel opme nts.(C) urbandehadingjia ngjun roa d,bri dgeand hig hway bri dges,thegreencityand other urba處在最初的探索階段。為了更加科學(xué)、嚴謹?shù)剞k好這個創(chuàng)新性的專業(yè)，我院于2008年 7月 31 日邀請了國內(nèi)知名醫(yī)學(xué)院校的教授、專家、學(xué)者及衛(wèi)生部、省衛(wèi)生廳和上饒市的相關(guān)領(lǐng)導(dǎo)來我院對我們的前期工作進行評估、論證， 希望他們對我們的后續(xù)工作提出指導(dǎo)

25、性的意見和建議。此次論證會邀請了衛(wèi)生部科教司孟群司長；首都醫(yī)科大學(xué)崔樹起教授；中國全科醫(yī)學(xué)雜志社劉海波教授；上海交通大學(xué)施榕教授；浙江大學(xué)余海教授；南昌大學(xué)袁兆康教授；人民軍醫(yī)出版社王少浪教授；省教育廳聶威處長；省衛(wèi)生廳李志剛處長。論證會的召開非常成功，得到了新聞、媒體和社會的高度關(guān)注，更得到了參會的各位教授、專家和領(lǐng)導(dǎo)們的高度評價和一致認可。當(dāng)然，各位專家、教授也對我們的教學(xué)大綱和教學(xué)計劃內(nèi)容提出了許多中肯而寶貴的意見，現(xiàn)在， 課題組根據(jù)專家、教授們的意見和建議，正在進行第三稿的修訂與完善。二、指導(dǎo)思想與目標(biāo)任務(wù)（一）指導(dǎo)思想堅持以鄧小平理論、三個代表和科學(xué)發(fā)展觀為指導(dǎo)，認真貫徹新時期衛(wèi)生工

26、作方針，以社區(qū)衛(wèi)生服務(wù)需求為導(dǎo)向，以提高社區(qū)衛(wèi)生服務(wù)水平和質(zhì)量為中心，按照長遠發(fā)展與當(dāng)前需要相結(jié)合和實事求是，突出重點，分類指導(dǎo)，逐步完善的原則， 以畢業(yè)后全科醫(yī)學(xué)教育為核心，以師資培訓(xùn)和崗位培訓(xùn)為重點，積極探索，勇于實踐，到2011 年逐步建立起符合我市實際較為完善的全科醫(yī)學(xué)教育體系，培養(yǎng)一支高素質(zhì)的數(shù)量充足的以全科醫(yī)師為骨干的社區(qū)衛(wèi)生服務(wù)隊伍，為我市社區(qū)衛(wèi)生服務(wù)發(fā)展提供可靠的人才保障。（二）發(fā)展目標(biāo)堅持以人的健康為中心，以向個人、家庭與社區(qū)提供連續(xù)、綜合、便利的基本衛(wèi)生服務(wù)為目標(biāo)，建立起適應(yīng)我市衛(wèi)生事業(yè)改革與發(fā)展需要的全科醫(yī)學(xué)教育體系。 努力培養(yǎng)和造就一支立足于社區(qū)，為廣大居民提供基本衛(wèi)生

27、服務(wù)，能用生物心理社會醫(yī)學(xué)模式開展融預(yù)防、醫(yī)療、保健、康復(fù)、健康教育、計劃生育技術(shù)服務(wù)為一體的高素質(zhì)的社區(qū)衛(wèi)生服務(wù)隊伍。（三）具體任務(wù)1、 2009年，構(gòu)建全科醫(yī)學(xué)教育的基本框架。建立市全科醫(yī)學(xué)教育培訓(xùn)中心和各區(qū)全科醫(yī)學(xué)教育培訓(xùn)站、臨床培訓(xùn)和社區(qū)培訓(xùn)基地，加強師資隊伍建設(shè).2、 2010年，初步建立和完善全科醫(yī)學(xué)教育體系。加強市全科醫(yī)學(xué)教育培訓(xùn)中心、 臨床和社區(qū)培訓(xùn)基地建設(shè)探索培養(yǎng)高素質(zhì)全科醫(yī)師的有效途徑和模式，然后逐步總結(jié)推廣。3、 2010-2012年，完善全科醫(yī)學(xué)教育體系。1）制定全科醫(yī)學(xué)教學(xué)大綱和教學(xué)計劃，編寫講義、教材和實驗指導(dǎo)圍繞建立以農(nóng)村社區(qū)為導(dǎo)向的全科醫(yī)學(xué)教學(xué)教育模式的總目標(biāo)，

46、務(wù)現(xiàn)狀調(diào)查7）赴有關(guān)院校和社區(qū)考察并寫出考察報告和向衛(wèi)生部請示報告和可行性報告。三、發(fā)展全科醫(yī)學(xué)教育的具體措施1）改革培養(yǎng)目標(biāo)與能力要求：以培養(yǎng)具有為人民健康服務(wù)的職業(yè)道德，“下的去、用的上、留的住,干的好”的農(nóng)村社區(qū)實用型全科醫(yī)學(xué)人才為目標(biāo)。其專業(yè)能力要求為：掌握全科醫(yī)學(xué)的基本理論、基礎(chǔ)知識和基本技能，熟悉全科醫(yī)療的診療思維模式，具備對社區(qū)常見、多發(fā)病、 傳染病健康問題和疾病的防治能力，能夠運用生物心理社會醫(yī)學(xué)模式， 以維護和促進健康為目標(biāo)，向個人、 家庭、社區(qū)提供醫(yī)療、預(yù)防、 保健、 康復(fù)、 健康教育、計劃生育指導(dǎo)為一體的衛(wèi)生服務(wù)，達到全科醫(yī)師崗位的基本要求。2）革新培養(yǎng)模式：根據(jù)培養(yǎng)目標(biāo)

47、和能力要求，將過去那種學(xué)校教育加醫(yī)院實習(xí)的培養(yǎng)模式改為：80 周在校教育二級以上醫(yī)院見習(xí)4 周醫(yī)院輪科實習(xí)40 周防疫站6 周（防疫科和衛(wèi)生科）社區(qū)實踐6 周3）優(yōu)化課程設(shè)置：根據(jù)面向農(nóng)村社區(qū)全科醫(yī)學(xué)人才培養(yǎng)不要求學(xué)生有十分寬廣的基礎(chǔ)理論知識，我們通過減除與基層臨床意義不大的公共基礎(chǔ)課（物理、化學(xué)、細胞生物學(xué)等）和壓縮公共基礎(chǔ)課（兩課、英語）就節(jié)剩出230 課時用于專業(yè)基礎(chǔ)課和專業(yè)課教學(xué)。同時根據(jù)“六位一體”功能要求增設(shè)預(yù)防、保健與健康促進等課程。見附表， 經(jīng)優(yōu)化設(shè)置后公共基礎(chǔ)課、專業(yè)基礎(chǔ)課和專業(yè)課所占必修課程的比重分別為16.4%、 32.8%和50.8%，而本科院校對應(yīng)的比例分別為30.7

48、%、 27.1%和 41.5%； 且專業(yè)課程無論從門數(shù)和時數(shù)與本科院校相差不大，這樣就從學(xué)時上保證學(xué)生的專業(yè)能力與質(zhì)量。4）改革教學(xué)內(nèi)容增強實用性：根據(jù)農(nóng)村社區(qū)全科醫(yī)師的能力要求重新編寫教學(xué)大綱和教材（講義）。刪除基礎(chǔ)課程中與基層社區(qū)衛(wèi)生工作無太大實際意義的臨床反復(fù)驗證性實驗內(nèi)容如彌漫性血管內(nèi)凝血試驗等等，增設(shè)三大常規(guī)檢查、心電圖機、快速末梢血糖測定、常見病X 片拍攝等臨床實用操作實驗；現(xiàn)行教材中相互重復(fù)的內(nèi)容則在某一課程中單獨講述，如現(xiàn)行內(nèi)科教材中已講水、電解質(zhì)與酸堿失衡，則在外科學(xué)教材中避免重復(fù)出現(xiàn)等等；新教材（講義）編寫中強調(diào)農(nóng)村常見病、多發(fā)病的診斷與防治；這樣精編實用性的教學(xué)內(nèi)容確保了
