12、求，對(duì)委托方提供的樣品進(jìn)行檢測(cè)，出具檢測(cè)報(bào)告。委托方根據(jù)本協(xié)議書要求提供可供檢測(cè)的有效樣品，按約定時(shí)間、金額支付檢測(cè)費(fèi)用。二、委托方通過(guò)自送、委托或郵寄等方式提供，供檢測(cè)的樣品應(yīng)是完整的、無(wú)破壞的、有效的、可用于檢測(cè)的；委托方向本站提供的一切資料均應(yīng)真實(shí)、完整、合法、有效；保證樣品可在國(guó)內(nèi)外合法銷售、流通，不用于非法的目的。委托方提供的檢測(cè)樣品如用于特殊目的，應(yīng)在簽訂本協(xié)議前提出。三、委托方對(duì)檢測(cè)報(bào)告有特殊要求，如對(duì)檢測(cè)結(jié)果有測(cè)量不確定度的評(píng)定等，應(yīng)在本協(xié)議書中提出。四、本協(xié)議書只接受委托方的書面方式提出的更改。由委托方代表簽字，經(jīng)本站重新進(jìn)行合同評(píng)審后認(rèn)為可行才進(jìn)行更改。更改后即按更改后的協(xié)

13、議內(nèi)容執(zhí)行。五、由本站確定的檢測(cè)方法或檢測(cè)方法有偏離時(shí)應(yīng)及時(shí)通知委托方，并得到委托方的確認(rèn)。六、本站按委托方的要求方式發(fā)放檢測(cè)報(bào)告。委托方自取，可憑本協(xié)議書或單位介紹信、聯(lián)系人身份證復(fù)印件等其他有效憑證領(lǐng)取。七、委托方如對(duì)檢測(cè)報(bào)告結(jié)果有異議，應(yīng)及時(shí)向本站提出。八、委托方不得利用檢測(cè)報(bào)告從事非法活動(dòng)，不得私自更/涂改。對(duì)上述行為造成的后果本站不承擔(dān)任何法律責(zé)任，并保留追究法律責(zé)任的權(quán)力。九、委托方自行復(fù)復(fù)制的檢測(cè)報(bào)告，未加蓋本站紅印章的，本站不予認(rèn)可。委托方如需多份檢測(cè)報(bào)告，應(yīng)酬在本協(xié)議書中同時(shí)提出。十、本站作為具有獨(dú)立法人資格的第三方公正、權(quán)威檢測(cè)機(jī)構(gòu)，所出具的檢測(cè)報(bào)告是完全基于第三方公正立場(chǎng)

14、，不受任何一方的干預(yù)、影響，對(duì)檢測(cè)結(jié)果負(fù)責(zé)，并對(duì)涉及委托方檢測(cè)技術(shù)數(shù)據(jù)、商業(yè)數(shù)據(jù)、知識(shí)產(chǎn)權(quán)、產(chǎn)品性能等信息保守秘密。十一、如遇火災(zāi)、水災(zāi)、爆炸、地震等人力不可抗拒的情況，造成樣品損壞、遺失，本站不承擔(dān)對(duì)樣品的損壞、遺失及檢測(cè)結(jié)果的責(zé)任，并可根據(jù)情況取消或延緩執(zhí)行協(xié)議。十二、除委托方在本協(xié)議中特別提出，本站對(duì)檢測(cè)樣品（除檢測(cè)中消耗外）保管至報(bào)告發(fā)出后30天。委托方愈期未領(lǐng)取的，本站將視委托方放棄并代為處理。十三、本協(xié)議書一式兩份，本站與委托方各執(zhí)一份。 terms and conditions for testing1. according to the requirement of the c

15、lient, the tester test the samples offered by them and issued the test report. the client shall legally provide the testing sample and pay the test fee at the arranged time as stipulated in the contract.第二聯(lián) 協(xié)議條款2. the sample offered by client can be sent by themselves, entrust or by mail, the sample

16、s should be complete, non-destroyed, effective and fit for testing, the information offered by client should be true, complete, legal and effective. it can be sold at home and abroad and cant be used for illegal purposes. if the tested sample will be used for special uses, notification should be men

17、tioned before signing the contract.3. the client shall notify tester by advance notice written in the contract if they have special requirements, such as evaluating the measurement uncertainty if the test result.4. the contract may only be modified by a variation expressed in writing on behalf of th

18、e client, signed by entrust, and confirmed by entrust after the review of the contract after identify it is feasible. it should be carried out according to the modified version after modification. 5. zfpts should notify the client in time when some deviations exist in the test result according to th

19、e testing method confirmed by zfpts, and confirmed by entrust.6. zfpts will issue test report according to the requirement of entrust. entrust can take the test report by themselves. if they missed the contract, they can take the introduction letter of the unit or the photocopy of contract persons i

20、d or other credit to take the test report.7. the client shall notify in time if they disagree with the test results.8. the client shall not make any illegal activity and make any correction and amendment of the test report. entrust wont liable for any responsibility caused by above actions and will

21、reserve the right to the legal responsibility.9. entrust will not confirm those test report if the client reproduce except in full without prior authorization from zfpts and have no red chop signed on it. if entrust needs several copies of the test report it should be mentioned in the contract at th

22、e same time.10. entrust is the third party of impartial and authoritative test organization with the qualification of independent juridical person, issue test report entirely based on the third party impartial ground and shall not be interfered and affected by any party, make serious responsibility concerning the test result, and keep secrets of the information of the en
