不同劑量順式阿曲庫銨對小兒肌松作用比較

1、 摘 要 目的：比較不同劑量順式阿曲庫銨在小兒外 科手術(shù)中的肌松作用，探討其臨床使用中最適劑量。方法： 48位患兒隨機分為3組，分別應(yīng)用2倍ED95 , 3倍ED95 和4 倍 ED95 的順式阿曲庫銨進行隨機對照研究，觀察 3 組 患兒肌松藥起效時間、有效作用時間、插管條件、血流動力 學等指標。結(jié)果：3組間注射順式阿曲庫銨前及注射15min后心率、平均動脈壓差異無統(tǒng)計學意義。 高劑量組（ 3 倍 ED95 和4倍ED95）順式阿曲庫銨起效時間、有效作用時間、體 內(nèi)作用時間均短于和久于 2倍 ED95 劑量組，差異有統(tǒng)計學 意義。 3 組間恢復(fù)指數(shù)差異無統(tǒng)計學意義。高劑量組能獲得 較好的插管條

At the same time， higher dose of cisatracurium had a better intubating condition than lower dose of cisatracurium， statistically. But no statistically significant difference was observed on onset time ， i

Conclusion ： The usage of 3xED95 and 4xED95 dose of cisatracurium provide a more effective and rapidly neuromuscular blockade in children with longer duration and hemodynamic hemostasis. 3x ED95 dose of cisatracurium i

7、s recommended as the initial dose.Key words cisatracurium ；doses；neuromuscularblockade；children由于小兒各項生理功能未健全，所以小兒手術(shù)麻醉對肌 松藥物選擇有較高要求。一些生理因素如神經(jīng)肌肉接頭發(fā) 育、肌肉纖維分布的不同及細胞外液容量的不同均影響非去 極化肌松藥起效及恢復(fù)的時間。所以，選擇更適合小兒特點 的肌松藥物， 選擇使用更適宜的劑量， 是臨床需探討的問題。 順式阿曲庫銨（cisatracurium , cis）是一種中時效的芐異喹 啉類非去極化肌松藥。是阿曲庫銨的一種光學異構(gòu)體，其肌 松作用約為

8、阿曲庫銨的 4倍，95%有效藥物劑量（ED95）為 0.05mg/kg，小于阿曲庫銨的 0.20mg/kg，但其起效較慢1。 順式阿曲庫銨不釋放組胺，心血管反應(yīng)小，是較為理想的非 去極化肌松藥 2-3 。本研究通過比較 2 倍 ED95、3 倍 ED95、 4倍ED95順式阿曲庫銨應(yīng)用在小兒外科手術(shù)中相關(guān)指標， 尋找其最適使用劑量，為臨床提供理論依據(jù)。1 對象與方法1.1 一般資料選取 2011 年 11 月至 2013 年 12 月于我院就診，擬擇期 行氣管插管全麻下外科手術(shù)的 48 例患兒為研究對象。入選 患兒選擇年齡12-60個月，ASA評分為I級或II級。排除有 心血管、肝、腎或神經(jīng)精

9、神系統(tǒng)癥狀或體征的患兒，以及正 在使用已知與順式阿曲庫銨有相互作用的藥物，如抗生素 （氨基糖苷類和四環(huán)素類） 、抗抑郁藥、抗驚厥藥、抗心律失常藥（鈣通道阻滯劑和奎尼丁）和硫酸鎂納入研究的 48 位患兒被隨機、平均分為 3 組，每組 16 人， 3 組患兒基線資料差異無統(tǒng)計學意義。分別對第1、2、3 組的患兒應(yīng)用起始劑量為 0.1mg/kg(2 倍 ED95)、0.15mg/kg(3倍ED95 )、0.2mg/kg (4倍ED95 )的順式阿曲庫銨。試 驗所使用方法可能的益處和并發(fā)癥告知患兒的監(jiān)護人，簽署 知情同意書。2 Kirov K ， Motamed C ， Decailliot F ，e
