Purpose 目的To establish procedures of reporting/following up any kind of incident, in order to identify the root causes; taking preventative measures to avoid similar incidents happen repetitively in the same or other plant; as well as acquiring indemnity or compensation from i

Definitions 定義3.1 Incident 事件 : Work-related event(s) in which an injury or ill health (regardless of sever

6、ity) or fatality occurred, or could have occurred (Near miss). 發(fā)生或可能發(fā)生傷害、疾病( 不論嚴重程度) 或死亡的與工作相關的事件。3.2 Accident 事故 : A sudden, unintended work (include our product)-related event which results in injury (occupational sickness), and/or damage to property/environment/ a third party, and/or business int

7、erruption. 指意外的非故意發(fā)生的，造成了與工作及產(chǎn)品有關的人身(健康)傷害、財產(chǎn)損失、意外停產(chǎn)、環(huán)境或第三方遭受損害后果的事件。3.3 Near miss 未遂事件: A sudden, unintended work -related event with no consequence, but this under slightly different circumstances could have become an accident. 指意外發(fā)生的，非故意并與工作有關，未造成不良后果，如果條件略微改變，有可能成為事故的事件。3.4 General near miss 一般未

8、遂事件： The risk assessment score of near miss is below 32風 .險評估分數(shù)低于32 分的未遂事件。3.5 Serious near miss 嚴重未遂事件： The risk assessment score of near miss is above orequal to 32. 風險評估分數(shù)大于等于32 分的未遂事件。4. Report Process報告流程4.1 Internal Report 公司內(nèi)部報告流程4.1.1 General Near Miss一般未遂事件Follow “P -HS-GE-008Near Miss and

9、Deviation .執(zhí)行公司程序”P-HS-GE-008NearMiss and Deviation。4.1.2 Serious near miss & Acciden嚴1 重未遂事件與事故4.1.2.1 All serious near miss and accidents must be communicated to company management team and HSE committee as Accident Notice within 2 hours by direct supervisor. 所有嚴重未遂事件和事故發(fā)生后，直接主管應在2小時內(nèi)向公司管理層和HS

10、E委員會發(fā)事故通知。4.1.2.2 Incident short report (the first two pages of -HS-GE-001 I“Fncident Report 事件 簡報 ” )must be sent by department supervisor/manager within 24 hours to company management team, HSE Committee. 直接主管/經(jīng)理應在24 小時內(nèi)完成事件簡報，并發(fā)送至公司管理層、HSE委員會。4.1.2.3 Investigation report must be sent by HSEwithin

11、 5 working days to company management team, HSEcommittee. HSE應當在5個工作日內(nèi)完成事件調(diào)查報 告，并發(fā)送至管理層、HSE委員會。4.2 External Report公司外部報告4.2.1 HSE should sent incident short report to CT-FM within 24hrs after incident . And sent investigation report within 10 working days after incident. HSE 應當在事件發(fā)生后 24 小時內(nèi)把事件簡報發(fā)鍛造機

12、加工集團。并且在10 個工作日內(nèi)發(fā)送事件調(diào)查報告。4.2.2 HSE should report to TKCN HSE Manager and Risk & Insurance Manager bysending accident short report within 1 working day of receiving. HSE 應當在收到事件簡報1工作日內(nèi)報告TKCN HS/理和風險&保險經(jīng)理。4.2.3 HR department should report accidents information into the PERSIS system monthly. H

13、R部門月度向PERS廢統(tǒng)報告。4.2.4 Company responsible person should inform Police/Safety Production Bureau should within one hour in case of fatal accident. Should also report to local quality supervision bureau if related special equipment. Contact information refers to “ Emergency Contact Lis,t see ” item 8.3.

14、 如發(fā)生死亡事故，單位負責人應在1 小時內(nèi)向警察及安監(jiān)部門報告。如與特種設備相關還應當報告當?shù)刭|監(jiān)部門。聯(lián)系 方式見8.3緊急聯(lián)系單。4.2.4.1 Report should include below content 報告應包括以下內(nèi)容:? Company name, address, type, productive and some basic information 事故發(fā)生 單位的名稱、地址、性質、產(chǎn)能等基本情況；? Time, location and situation of accident scene. 事故發(fā)生的時間、地點以及事故 現(xiàn)場情況；? The brief des

15、cription of accident (include emergency rescue). 事故的簡要經(jīng)過(包括應急救援情況)；? Measures already taken 已經(jīng)采取的措施。5.Cordon off the accident area and collect related information 保護現(xiàn)場和收集信息1.1 Area owner should take all necessary actions to prevent aggravation of loss 區(qū)域責任人 應當采取一切措施防止損失擴大。1.2 The direct leader shoul

16、d follow or arrange one employee to follow the employee to the hospital and take a picture/copy of doctor' s note and siCk鹿Oveno陪同或安排一名同事陪同受傷員工前往醫(yī)院治療，并獲取醫(yī)囑及病假條的照片或復印件。1.3 The direct leader should send information to HR department and HSE department in the same day直接主管應Ig在當天將5.2中的信息發(fā)送給HR部門與HSE部門。

17、1.4 HSE collect incident information such as incident time, place, course, injury state, medical statement, on-site evidence and photos. HSE 收集事件信息：時間、地點、經(jīng) 過、受傷情況、醫(yī)生診斷、現(xiàn)場物證及照片。1.5 Area owner should cordon off the accident area If need to recovery the site at once,the company shall get written permit

18、 from Government authorities, Insurance company and company management. Collect and keep all relevant evidences; take photos from different angles. 區(qū)域負責人應當保留事故現(xiàn)場。如須立即修復需獲得政府、保險公司或公司的允許，并收集所有相關證據(jù)，多方位拍照。6. Investigation 事件調(diào)查6.1 All accidents and serious near miss must be subject to an investigation to

19、 find out the root cause and write investigation report. 所有事故和嚴重未遂事件必須進行調(diào)查，找出根本原因并形成報告。6.2 Investigation Team 調(diào)查小組6.2.1 Investigation team must be established before investigation. 事件調(diào)查之前必須成立 調(diào)查小組。6.2.2 HSE should define the investigation team and lead the investigation. HSE 必須定義調(diào)查小組并且主導調(diào)查。6.2.3 For

20、 fatal accident, CEOshould lead the investigation and define the investigation team. 死亡事故，首席執(zhí)行官必須主導調(diào)查并且定義調(diào)查小組。6.2.4 Area owner, director leader, injured people if possible, witnesses, union member and related department leader should be involved in the investigation. 區(qū)域責任人、直 接主管、受傷人員、目擊者、工會成員以及相關部門主

21、管應參加事故調(diào)查。6.3 Investigation process調(diào)查流程6.3.1 Interview with injured people and eyewitnesses; 與受傷人員和目擊者面談；6.3.2 Hold meeting to discuss the sequence of events leading to the incident; 開會探討事 件；6.3.3 Document the sequence of events;記錄事件發(fā)生順序；6.3.4 Conduct a “ Why Analysis ” anorddrreecsult; 使用 “為什么分析法”并記

22、錄根本原因；6.3.5 Discuss necessary corrective actions to eliminate root causes; 討論消除根本原因必要措施；6.3.6 Assign members duties and responsibilities to carry out these corrective actions; 指 派整改責任人；6.3.7 Verify corrective measures and document all files.驗證整改措施并存檔。7.
