胰十二指腸切除術(shù)Partone——獻給丁香園外科同道1.A69-year-oldwomanhadanabdominalCTscanaspartofaworkupforproteinuria.Thescanshowedacysticlesionoftheheadofthepancreas.CT-guidedaspirationofthemassreturnedcuboidalandcolumnarepithelialcellswithpositivemucinstaining.Theamylasecontentofthefluidwaslow.2.TheCTscanalsoshowedmultiplecalcificationsthroughoutthebodyofthepancreas.Thepatienthadnohistoryofpancreatitisoralcoholabuse.Shehadnosymptomsofpancreaticexocrineinsufficiency,butshewasanon-insulin-dependentdiabetic.Theremainderoftheworkupwasnegative.4.AKochermaneuverwasperformed.TheperitonealreflectionontheconvexsideoftheC-loopwasincisedandtheduodenumandpancreaticheadwerebluntlymobilizedmediallyoffGerota'sfasciaandtheinferiorvenacava.5.Themobilizedpancreaticheadisshown.Therewasacleanseparationfromtheinferiorvenacava.Therewasnoadenopathyintheportahepatis.6.Thefourthportionoftheduodenumwasidentifiedbeneaththetransversemesocolon.Theinferiormesentericveinwasseenabovetheduodenum.7.ThepositionoftheIMVrelativetothefourthportionoftheduodenumisshown.Therewasnoevidenceoftumorinvolvementatthislocation.9.Thearterialsupplytothepancreasisshown.Theheadissuppliedbythepancreatoduodenalarcadesconnectingthehepaticarteryabovewiththesuperiormesentericarterybelow.Thetailissuppliedbybranchesofthesplenicartery.11.Theinferiorpancreaticoduod
