1、 國際金融學(xué)生論壇會員申請書 APPLICATION FORM FOR FINANCE INTERNATIONAL STUDENT SYMPOSIUM申請人須知 NOTES FOR APPLICANTS 以下備注為填寫申請書(A.M.FISS)提供指引。The following notes give guidance on how to complete the application form (A.M. FISS)(a) 申請人只能就此次項(xiàng)目填寫一份申請書。Applicant should complete one application form for this program.(b

2、) 申請人須填妥申請書各項(xiàng)，并提供正確數(shù)據(jù)。如空位不敷填寫，申請人應(yīng)另頁詳列有關(guān)資料，隨申請書附上。Please ensure that all parts in the form are completed and the information is accurate. If there is insufficient space, please give details on a separate sheet to be attached to the application form.(c ) 申請人必須填報(bào)申請書內(nèi)要求提供的所有個(gè)人資料(注明是可選擇是否填報(bào)的數(shù)據(jù)則屬例外)，否則申請

3、書將不獲受理。Your provision of all the personal data requested in this form is obligatory(except those items clearly marked as optional). You should particularly note that program may require you to provide specific details to support your application for individual vacancies. Your application will not be

4、 considered if you fail to provide all information as requested or it is not clearly.(d) 申請人在本申請書內(nèi)所提供的個(gè)人資料，將用于項(xiàng)目以及其他與有關(guān)的事宜上。如有需要，有關(guān)資料可能會送交獲授權(quán)處理有關(guān)數(shù)據(jù)的部門及其他組織或機(jī)構(gòu)，用以進(jìn)行與項(xiàng)目有關(guān)的事宜。在一般情況下，未獲取錄申請人的數(shù)據(jù)將于其落選日期后12個(gè)月全部銷毀。The personal data provided in this form will be used for the program and other program-related

5、 purposes. It may be provided to related departments and other organizations or agencies authorized to process the information for necessary purposes. Information on unsuccessful candidates will normally be destroyed 12 months after rejection of the candidates application.(e ) 填妥本表格后，你可保留副本一份，作個(gè)人參考之
