2、identifying PPE requirements as part of a workplace risk assessment.2. SCOPEThis procedure applies to all Xxx employees, contractors, temporary help, or visitors to Xxx facilities. It also applies to Xxx contract personnel and contract employees working at customer sites and supplier sites.3. SUMMAR
3、Y? Sites must conduct a risk assessment and define PPE requirements for specific tasks, activities, and locations/areas.? PPE is the last consideration in the hierarchy of protection of employees where other protective measures (such as engineering and administrative controls) have been implemented
4、and a workplace risk still remains.? Specify and use PPE in accordance with this procedure, the referenced procedures, any additional country-specific requirements, and the manufacturers instructions.4. PROCEDURE4.1 Responsibilities4.1.1 Site management is responsibletoensure the following:Workplace
5、 risk assessmentiscompleted.All personnel are trained and use PPE when required.Adequate supply and various sizes of PPE are available and that PPE is properly maintained.01.05.06 , Rev. 3, Page 7 0f 74.1.2 Site personnel are responsible for? Wearing suitable, proper-fitting PPE as determined throug
6、h a workplace risk assessment ? Maintaining PPE.? Informally evaluating tasks and areas during the course of their work activities and applying the principles associated with this standard to all work situations.4.2 Workplace Risk Assessment4.2.1 Risk assessments identify any need for PPE. A site su
7、rvey and review of all the workplace assessments will be carried out to determine the tasks and the areas of plants which require PPE.4.2.2 Assessment MethodThe following method(s) may be used to complete and communicate the site/area risk assessment.?Process Hazards Assessment?Design Hazards Review
8、?Safe Work Practice?Job Safety Analysis?Standard Operating Procedure?PPE Matrix (Example in Appendix1)The Safe Work Permit is a method of specifying PPE for a specific task/activity for a specific period of time. However, the Safe Work Permit may not serve as your primary or only method of risk anal
9、ysis.The following methods(s) may be used to informally evaluate and communicate area or task risks.? Pre-task planning? Safety Contacts? Accident Predictive Techniques (APT)4.2.3 Assessment CriteriaAt a minimum, consider the following risks and risk sources to determine proper PPE for hazards assoc
10、iated with work tasks and areas: ? Impact? Penetration? Compression (Roll Over)? Chemical contact or exposure? Hot/Cold Temperatures?Harmful Dust? Light (Optical) Radiation?Sources of Motion?Types of Chemical Exposures?Falling Objects?Sharp Objects?Rolling or PinchingObjects(Foot)?Layout of the Work
11、placeand Location of Workers?Electrical Hazards?Flash fire potential4.2.4 Assessment CertificationIn the U.S., sites are also required to complete the certification form found in The US Attachmentto this Standard (01.05.06 AM Personal Protective Equipment OverviewU.S. Attachment)4.3 Selection of PPE
12、The PPE must be designed and rated to protect the specified body part(s) against the hazard(s) identified in the risk assessment or informal area/task evaluation. These requirements must be made clear to all employees, contractors, or visitors affected.4.4 Specification and PurchaseA clear and detai
13、led specification must be provided to all vendors for the PPE to be purchased from them. Advice should be sought from Xxx EH&S or other expert personnel when drawing up this specification. Where available, centrally negotiated contracts and/or country-specific purchase practices will be used for
14、 the purchase of PPE.4.5 Training4.5.1 Personnel must be trained in the need for and the use of the PPE issued.4.5.2 User training will cover the following:? An explanation of the risks and why PPE is needed? The operation, performance, and limitations of PPE? Instructions on the selection, use, and
15、 storage? Factors which can affect the protection provided by PPE such as other protective equipment, personal factors; working conditions; inadequate fitting; damage; defects, and wear? Recognizing defects and arrangements for reporting loss or defects? Replacement? Disposal? Practice in putting on
16、, wearing, adjusting, and removing PPE, and demonstration of usingPPE properly? Practice and instruction in inspection and maintenance, and where appropriate, testing of PPE before use? Instruction in safe storage of PPE4.4.3 Records of training and refresher training must be kept by the facility ma
17、nager or a person at the site authorized to manage the training records. A training record must include the names, date, subject/content of training, and the method used to demonstrate competency or knowledge. The records serve as certification of training.Refresher TrainingRefresher training must b
18、e conducted when:Additional work areas or new or added tasks requiring PPE render previous training obsolete.Changes in the types of PPE used renders previous training obsolete.? Inadequacies are observed in the affected personnel's knowledge or use of PPE.4.6 SignsDesignate PPE areas with signs
19、 or other visual markers that specify the mandatory type of PPE required to be worn in that area, e.g., hard hat, eye protectors, ear protectors, etc., based on Appendix 1. Also, designate these areas on Site plot plans.4.7 Storage and MaintenanceEach facility must provide adequate storage for the r
20、equired PPE to provide protection from contamination, loss, or damage.Maintain PPE in accordance with the manufacturer's instructions.Define the responsibilities and frequencies for carrying out maintenance (including washing).? Simple maintenance, such as checking that the PPE is free from defe
21、cts prior to use, may be carried out by the wearer? More complex maintenance such as checking a chemical suit for leak tightness must be carried out by trained and qualified staff, and such PPE will be included in the site's Preventative Maintenance Program.PPE which cannot be decontaminated mus
22、t be disposed of in a manner that protects personnel from exposure to hazards and protects the environment.4.8 ReplacementAll types of PPE have a limited life that may depend on the work conditions, frequency of washing, general wear and tear, and manufacturer specifications.Manufacturers' instr
23、uctions that define the life of an article of PPE must be followed.Where no life is stated by the manufacturer, the PPE shall be replaced on the basis of normal wear and tear4.9 CompatibilityWhere more than one workplace risk is identified, there may be a requirement for the use of more than one typ
24、e of PPE. A typical example may be the requirement to protect against inhalation of dusts and protection from falling objects. In this type of situation the two types of PPE, i.e., respirator and head protection must be considered together and must ensure that they do not interfere with each other
25、39;s functions.4.10 Visitors and ContractorsVisitors to the facility who are on a guided tour of the operational areas may not require the same level of PPE as those staff performing operational or maintenance activities; however, each facility must assess the risks to visitors and require the appro
26、priate level of PPE.Contractors who are working within the facility must comply with all site rules and wear the same level of PPE as operational staff. The contractor's management is responsible to ensure that all affected personal have completed PPE training. Xxx employees who visit a site mus
