1、運動員參賽保證書 - 保證書本人對參加此次賽做以下保證：1 、 我保證：遵守比賽組委會所有規(guī)則和服從一切裁決。2 、 我保證對自己的參賽器材、 裝備及個人所有物品負(fù)有全部責(zé) 任。3 、我對在連續(xù)而激烈的比賽中可能發(fā)生的危險已有充分的認(rèn)識， 我保證本人的身體健康狀況能夠承受本次比賽， 并具備近期健康檢查 證明書。4 、我保證：本人在比賽中因受傷、遭遇意外事故或發(fā)生疾病時， 對受到的急救處理方法及結(jié)果不提出異議。5 、我本人、我的家屬、親戚和保護(hù)人已充分理解本次比賽的內(nèi) 容和意義， 并認(rèn)可我本人參賽。 在比賽之前我已作了人身保險。 因此， 我保證：我自己、親屬及遺囑執(zhí)行人、財產(chǎn)繼承人，對本人在比賽

2、中 或參加由本次比賽委會組織的與比賽有關(guān)的活動時， 不論什么原因所 致的傷、病、亡或出現(xiàn)的后遺癥等，不對比賽組委會提出任何起訴。 所造成的損失由保險公司和本人承擔(dān)， 比賽組委會不再提供因此而造 成的損失費。6 、我保證：允許比賽組織者自由使用和制作與比賽有關(guān)的我的 名字、照片等。并承認(rèn)與本次比賽有關(guān)的新聞、廣播、電視等商業(yè)宣 傳報道。7 、我保證：由于自然災(zāi)害等客觀原因造成比賽取消或變更比賽 時間、內(nèi)容所引起的有關(guān)費用問題，服從比賽組委會的決定；由于我 個人的情況而不能參加比賽的，不要求退還參賽時所交納的各種費 用。8 、我確認(rèn)：對參賽所進(jìn)行的上述保證已充分理解，并承認(rèn)本保 證書解釋權(quán)屬比賽組
